Does Quercetin Work? Blood Pressure, Allergies, and Absorption Issues

Plain quercetin modestly, measurably lowers blood pressure at 500 mg/day and up — three independent meta-analyses agree, and it's the strongest thread in this category. Allergy relief is a real mechanism with thin trial support (one positive phytosome RCT, n=66). The absorption gap between forms — phytosome reaches up to 20x higher plasma levels than plain at the same mg (Riva 2019) — is the single most buyer-relevant fact in the category.
Four claims, graded on their own evidence — not on each other
Quercetin marketing routinely blends a real blood-pressure finding, a thin allergy signal, a mixed exercise-immunity picture, and an absorption story into one confident-sounding "powerful antioxidant" pitch. Separated honestly, they're four very different evidence states.
1. Blood pressure — real, modest, and dose-dependent (the strongest claim)
Three independent meta-analyses — Serban 2016 (7 RCTs, n=587), Huang 2020 (17 RCTs, n=896), and Popiolek-Kalisz 2022 (10 RCTs, n=841) — each found a small, statistically significant systolic/diastolic reduction from plain quercetin, roughly 2–4.5 mmHg systolic, larger in people who already run hypertensive or pre-hypertensive. Serban's dose-stratified data draws a hard line: significant at 500 mg/day and up, not significant below it. 2. Allergic rhinitis — real mechanism, thin trial base
Quercetin has genuine mast-cell-stabilizing, antihistamine-adjacent activity. One randomized trial (Yamada 2022, n=66, 200mg/day Quercetin Phytosome, 4 weeks) found significant improvement in eye itching, sneezing, nasal discharge, and sleep disturbance versus placebo — industry-associated branded ingredient (Quercetin Phytosome/Quercefit); self-reported symptom scores. A pediatric pilot (Tosca 2025) reported "favorable" results for a multicomponent quercetin-phytosome-plus-zinc-and-vitamin-C formula, but its abstract wasn't available to extract numbers from, and it's confounded by the added ingredients — cite it as a signal only, never with invented specifics. Compared to the blood-pressure thread's three meta-analyses and dozens of pooled RCTs, this is a fraction of the trial volume. "Mast-cell-stabilizing mechanism with early clinical support" is accurate; "natural antihistamine" as a settled fact is not.
3. Exercise and immune support — mixed, and a textbook case for reading both trials together
Nieman 2007 (n=40 trained cyclists, 1,000mg/day quercetin around intensified training) found significantly fewer upper respiratory infections in the quercetin group (1 case vs 9 on placebo) in the two weeks after training — but immune markers like NK cell activity and salivary IgA did not differ between groups despite that illness difference. Henson/Nieman 2008 — same lab, same 1,000mg/day dose, ultramarathoners instead of cyclists — found NO protective effect at all: illness rates, granulocyte respiratory burst, and salivary IgA were unchanged. Citing the 2007 result without the 2008 null result would misrepresent this thread as more consistent than it is. A broader meta-analysis of flavonoids generally, not quercetin specifically (Somerville 2016, 14 studies), found a 33% URTI reduction but "trivial" biomarker differences — supporting context for the mechanism class, not quercetin-specific proof.
4. The absorption problem — the single most buyer-relevant fact in this category
Plain quercetin is poorly absorbed. Riva 2019, a randomized crossover pharmacokinetic trial (n=12), found phytosome (lecithin-bound, Quercefit) quercetin reaches plasma levels up to 20 times higher than plain quercetin at the SAME labeled mg. This is why phytosome brands can charge more per labeled mg and still be a defensible choice for some goals — and it's why the blood-pressure trials (plain, 500–1000mg/day) and the allergy trial (phytosome, 200mg/day) land on such different-looking numbers. The verdict Quercetin's best-supported claim is a modest, dose-dependent blood-pressure reduction at 500 mg/day and up (plain form). Allergy support is real-mechanism but thin-trial. Exercise/immune evidence is genuinely mixed. None of this supports a disease-cure or anti-aging claim — it supports a real, modest, form-dependent supplement, and the form you buy changes what dose actually means.
