Flaxseed Oil vs Fish Oil vs Algal Oil: The ALA Conversion Problem

Flaxseed oil delivers ALA, and ALA converts to EPA at roughly 5% and to DHA at under 0.5% in healthy nonvegetarian adults (Plourde & Cunnane 2007). That's the hard biochemical wall the "flaxseed oil is a vegan fish-oil substitute" pitch runs into: it reliably raises blood EPA, modestly, but it does not raise DHA — confirmed across every multi-week human dosing trial in this evidence base. A 2022 scoping review of 13 RCTs found the honest verdict in plain language: "High dose flaxseed or echium seed oil supplements provided no increases to O3I [omega-3 index] and some studies showed reductions... microalgal oil supplementation increased O3I levels for all studies" (Lane et al. 2022). If EPA+DHA or the omega-3 index is the goal, algal oil is the vegan source that actually does that job.
The conversion-rate ceiling: the numbers behind the wall
The core mechanism is a short, capacity-limited enzyme pathway: ALA → (desaturation/elongation) → EPA → DPA → DHA. Plourde & Cunnane 2007, a narrative review, states the real-world dietary-supplementation figure plainly: "≈5% of ALA is converted to EPA and <0.5% of ALA is converted to DHA... even large amounts of dietary ALA have a negligible effect on plasma DHA." Isotope-tracer studies under controlled single-dose conditions show a more optimistic ceiling: Burdge & Wootton 2002 found young women's net fractional conversion reached an estimated 21% EPA and 9% DHA-pathway intermediate — but the companion study in young men, Burdge, Jones & Wootton 2002, found "no apparent 13C enrichment of docosahexaenoic acid (DHA)... at any time point measured up to 21 d" — zero detectable DHA synthesis from ALA in young men over three weeks. The authors' own conclusion: "the capacity of adult males to convert ALNA to DHA was either very low or absent." Critically, these tracer percentages are net fractional inter-conversion estimates from a single controlled dose — a different measurement than what a daily tablespoon delivers over weeks of real use, where DHA essentially never rises (see below).
The real-world dosing-gap evidence: flax needs roughly double the dose fish oil needs
Barceló-Coblijn 2008 (n=62 firefighters, 6-arm dose-ranging RCT, 12 weeks) is the closest thing to a direct flax-vs-fish-oil trial in the human literature: arms received 1.2/2.4/3.6g ALA/day from flax oil, 0.6/1.2g EPA+DHA/day from fish oil, or a sunflower-oil placebo. Fish oil produced a rapid rise in erythrocyte DHA and total n-3, as expected. Flax oil at 2.4 or 3.6g ALA/day significantly raised erythrocyte ALA, EPA, and DPA — but the abstract reports no significant erythrocyte DHA rise in the flax arms. The authors' own conclusion: "The amounts of ALA required to obtain these effects are amounts that are easily achieved in the general population by dietary modification" — real, but note it took roughly double the ALA dose (in grams) that fish oil needed of EPA+DHA to approach comparable erythrocyte n-3 movement.
Kuhnt et al. 2016 sharpens this further: 5g ALA/day from linseed (flaxseed) oil was compared against fish oil, echium oil (which contains some pre-converted stearidonic acid), and olive oil across plasma, erythrocyte, and PBMC fractions. Echium oil raised EPA and DPA roughly 25% more than linseed oil; the relative effectiveness for raising EPA+DPA ran roughly 100:25:10:0 (plasma) for fish:echium:linseed:olive oil — linseed/flaxseed oil's EPA-raising effect was only about 10% as strong as fish oil's at these doses. The authors' own conclusion, quoted directly: "neither EO nor LO maintained blood DHA status in the absence of fish/seafood consumption" — flaxseed oil did not raise DHA, and neither did the more-effective echium oil.

Lane et al. 2022's scoping review screened 639 articles down to 13 RCTs (Jan 2010-Sep 2020) comparing plant-based n-3 oil sources (flaxseed, echium, microalgal) using the omega-3 index (O3I = EPA+DHA%) as the outcome measure. Their verbatim conclusion: "High dose flaxseed or echium seed oil supplements, provided no increases to O3I and some studies showed reductions. However, microalgal oil supplementation increased O3I levels for all studies." Their own recommendation follows directly: "regular consumption of preformed EPA and DHA supplements may help maintain optimal O3I" for vegetarians and vegans — i.e., the reviewers themselves point to algal oil, not flaxseed oil, as the path to raising the omega-3 index. This is a scoping review (breadth over depth, no formal risk-of-bias meta-analytic pooling), but it is the cleanest, most direct verdict in this cluster's entire evidence base, in the reviewers' own words, not this site's paraphrase.

If the goal is raising EPA+DHA blood levels or the omega-3 index, algal oil is the better vegan choice — it delivers preformed EPA/DHA directly and raised O3I in every study Lane 2022 reviewed. If the goal is simply adding ALA to the diet — its own nutrient, with a real, if modest, observational CVD association (Pan 2012) — at low cost, flaxseed oil is a reasonable way to do that. The two goals are not interchangeable, and flaxseed oil should never be marketed as "just as good as fish oil" or "your DHA source."
