Algal Omega-3: The Vegan EPA+DHA Source, What Trials Show

Algae is the original source of marine omega-3 — fish are the middleman. Fish don't make EPA/DHA; they eat algae, or things that ate algae, and bioaccumulate it. So when a vegan buyer asks whether algal oil is really equivalent to fish oil, the honest answer is yes, on the metric that matters for absorption: Arterburn 2008 ran algal-oil DHA capsules head-to-head against cooked salmon and found them "nutritionally equivalent" — DHA rose the same amount in both groups. What algal omega-3 is, and the four threads that hold up
Algal oil is EPA and/or DHA extracted directly from cultivated marine microalgae (commonly Schizochytrium or Ulkenia species) rather than from fish tissue — the same biosynthetic origin fish rely on, just without the fish. Doughman 2007, a narrative review, frames this plainly: algae synthesize EPA/DHA; fish accumulate it up the food chain. That's a different picture than flaxseed, chia, or walnut oil, which deliver ALA — a plant omega-3 that converts poorly to EPA/DHA in the human body (Santos 2020). Algal oil delivers preformed EPA/DHA directly, which is why it can be compared to fish oil at all.
Four threads of verified human trial data run through this evidence base. First, bioequivalence: Arterburn 2008 (n=32, 2 weeks) gave healthy adults 600mg DHA/day from either algal-oil capsules or assayed portions of cooked salmon — DHA rose ~80% in plasma phospholipids and ~25% in erythrocytes in both groups, statistically equivalent by the authors' own bioequivalence analysis. A companion trial, Arterburn 2007 (n=12/group, 28 days), dose-ranged two algal DHA oil strains against a DHA-fortified food and found linear, dose-dependent, statistically equivalent uptake across all three delivery forms. Both trials are affiliated with Martek Biosciences, the DHASCO algal-oil manufacturer — a real conflict of interest, disclosed here every time these trials are cited. Second, triglyceride-lowering: Geppert 2006 (n=114 vegetarians, 8 weeks, EPA-free algal DHA) cut triglycerides 23% (p<0.001), and Maki 2014 (n=93 hypertriglyceridemic adults) found a 2.4g/day algal DHA+EPA blend produced an 18.9% TAG reduction — not statistically different from a 2.0g/day fish-oil comparator's 22.9% reduction. Third, and this is the honesty thread that must travel with the second: LDL-cholesterol rises alongside the triglyceride benefit in nearly every algal-DHA trial. Bernstein 2012, a meta-analysis pooling 11 RCTs (n=485), found algal DHA lowered triglycerides and raised HDL-C, but also raised LDL-C by a pooled +0.23 mmol/L (roughly +9mg/dL) — Harvard-affiliated authors with no apparent industry association, which strengthens rather than weakens this finding. Fourth, the vegan/vegetarian-population evidence: Conquer & Holub 1996 (n=24 vegetarians, DHA-only oil) found serum DHA rose 246% over 6 weeks, and Sarter 2015 found a low-baseline subgroup of vegans raised their omega-3 index from 3.1% to 4.8% on a modest 254mg/day dose — real, but still under the 8% cardiovascular target used on this site's own omega-3 index test page.
Does it work? (the honest short version)
On raising blood DHA and moving lipid biomarkers, yes — reliably and reproducibly. On preventing a heart attack or stroke, algal oil has never been directly tested, and that distinction has to stay explicit every time this evidence gets summarized. The bioequivalence data (Arterburn 2008, 2007) is the closest thing to a direct "vs. real fish" comparison in the human literature, and it holds up. The triglyceride-lowering data (Geppert, Maki) is real and, in Maki's head-to-head, statistically indistinguishable from fish oil at matched high doses. But every landmark cardiovascular-outcome trial that news coverage cites for omega-3s — VITAL, ASCEND, REDUCE-IT, GISSI-Prevenzione — dosed fish-derived EPA/DHA, never algal oil. Algal oil's case is bioequivalence plus biomarker movement, extrapolated from the premise that once DHA/EPA is in the bloodstream, the body doesn't know which organism made it. That premise is biologically reasonable and the bioequivalence data supports it, but it is an extrapolation, not a hard-outcome finding, and "raises your omega-3 index the same as fish oil" is the correct claim — "prevents heart attacks the same as fish oil" is not.
