Flaxseed Oil for Heart Health: Blood Pressure and Cholesterol Trials

The honest answer
Two small, real RCTs found flaxseed oil moved a cardiovascular biomarker: Paschos 2007 found 8g ALA/day significantly lowered blood pressure at 12 weeks (n=59), and Kawakami 2015 found 5.49g ALA/day significantly lowered small dense LDL (n=15). Both are real findings, both are small trials in narrow populations. Paired with them: Pan 2012's meta-analysis found ALA's own cardiovascular-risk association is modest and observational, not RCT hard-outcome evidence — there is no ALA equivalent of VITAL, ASCEND, REDUCE-IT, or GISSI-Prevenzione. Never read "lowered blood pressure in one trial" as "prevents heart disease."
Blood pressure: Paschos 2007
Paschos et al. 2007 (n=59 middle-aged dyslipidaemic men, prospective parallel-arm RCT, 12 weeks) compared 8g/day ALA from flaxseed oil against a safflower-oil control providing an equivalent 11g/day linoleic acid. PRIMARY endpoint (blood pressure): MET — "Supplementation with ALA resulted in significantly lower systolic and diastolic blood pressure levels compared with LA (P=0.016 and P=0.011, respectively)." The authors' own framing matters here: they describe this as a "hypotensive effect of ALA, which may constitute another mechanism accounting in part for the apparent cardioprotective effect" — hedged language, not a firm causal claim, and this site doesn't upgrade it to one. The trial population was middle-aged dyslipidaemic men only; the result isn't generalizable to women or normotensive/normolipidemic adults without that caveat. This is the highest ALA dose in this cluster's evidence base — well above what a single retail serving delivers (see the dosage guide).
Small dense LDL: Kawakami 2015
Kawakami et al. 2015 (n=15 Japanese men, randomized double-blind crossover, 12 weeks per arm) compared 10g/day whole flaxseed oil (delivering 5.49g ALA) against 10g/day corn oil. PRIMARY endpoint (serum small dense LDL, sd-LDL — a more atherogenic LDL subfraction than standard LDL-C): MET — sd-LDL significantly decreased at both 4 and 12 weeks with flaxseed oil, with no change on corn oil; the effect was more pronounced in subjects with baseline triglycerides over 100mg/dL. Plasma ALA and EPA rose significantly, but plasma DHA showed no significant difference between periods — the same EPA-rises/DHA-doesn't pattern that runs through this entire cluster. CETP and apoB were also significantly lower in the flax period at 12 weeks. This is the best available lipid-subfraction RCT in this evidence base, funded by the University of Tokushima (academic, no apparent industry association from the listed author affiliations) — but the trial is very small (n=15), single-sex, single-ethnicity, and crossover-designed, which limits how far the finding generalizes.

The honest ceiling: Pan 2012's meta-analysis
Pan et al. 2012's systematic review and meta-analysis (27 observational studies, n=251,049, 15,327 CVD events, Harvard-affiliated authors with no apparent industry association) found ALA intake associated with a pooled RR of 0.9 (95% CI 0.81-0.99) in dietary-intake studies — a real, modest association. But the biomarker-based studies (blood/tissue ALA level, arguably the more objective exposure measure) showed a similar-direction trend that was not statistically significant (pooled RR 0.8, 95% CI 0.63-1.03), and overall between-study heterogeneity was high (I²=71.3%). The authors' own conclusion: "higher ALA exposure is associated with a moderately lower risk of CVD... but the high unexplained heterogeneity highlights the need for additional well-designed observational studies and large randomized clinical trials." This is purely observational evidence — there is no ALA/flaxseed-oil equivalent of VITAL, ASCEND, REDUCE-IT, or GISSI-Prevenzione, all of which dosed marine EPA/DHA in hard-outcome RCTs. State any CVD-risk claim for flaxseed oil at this level: "modestly associated with lower CVD risk in observational dietary studies," never "prevents heart disease" or "as protective as fish oil."
