Flaxseed Oil: What the Human Conversion Studies Actually Show

Flaxseed oil reliably raises blood EPA — it does not raise DHA. The ALA→EPA conversion pathway runs at roughly 5%, but ALA→DHA runs at under 0.5% in healthy nonvegetarian adults (Plourde & Cunnane 2007), and DHA essentially never moves in real multi-week dosing trials, no matter the dose. That makes flaxseed oil a genuine, if modest, way to raise EPA and add dietary ALA — not a way to raise DHA or replicate what marine or algal oil does. What flaxseed oil is, and the four threads that hold up
Flaxseed oil is the pressed liquid fat of the flax plant (Linum usitatissimum), delivering alpha-linolenic acid (ALA) — the short-chain plant omega-3. It is not the same product as ground or whole flaxseed, which also carries lignans (phytoestrogens) and fiber concentrated in the seed coat that pressing leaves behind; hormone-sensitive-condition claims tied to lignans belong to the seed, not the oil, and shouldn't be imported here. The marketing pitch — "a vegan way to get your omega-3s" — runs into a biochemical wall the moment you ask which omega-3: humans can convert ALA into EPA and DHA, the two long-chain omega-3s with the real cardiovascular and cognitive evidence base, but the pathway is short, capacity-limited, and gets worse at every step.
First, the conversion-rate ceiling. Plourde & Cunnane 2007, a narrative review, states it plainly: "≈5% of ALA is converted to EPA and <0.5% of ALA is converted to DHA" in healthy nonvegetarian adults — the single load-bearing quantitative citation for this entire cluster. Isotope-tracer studies show a more optimistic picture under controlled single-dose conditions: Burdge & Wootton 2002 found young women's net fractional conversion reached 21% EPA, 9% DHA-pathway — but the companion study in young men, Burdge, Jones & Wootton 2002, found no detectable DHA synthesis at all over 21 days. These tracer percentages describe net conversion capacity from a single dose, not what a daily tablespoon actually delivers — don't conflate the two.
Second, the EPA-rises/DHA-doesn't pattern shows up in every real dosing trial. Harper 2006 (n=56, RCT) found plasma EPA rose 60% on 3g ALA/day (p=0.004) — but plasma DHA did not change in either group. Barceló-Coblijn 2008 (n=62 firefighters, dose-ranging RCT) found 2.4-3.6g ALA/day raised erythrocyte EPA and DPA, with no significant DHA rise reported in the flax arms — and it took roughly double the ALA dose to approach what a comparably-dosed fish-oil arm achieved. Kawakami 2015 (n=15 crossover) found the same pattern: ALA and EPA rose, DHA showed no significant difference.
Third, the honest cardiovascular-risk ceiling. Pan et al. 2012's meta-analysis (27 observational studies, n=251,049, 15,327 CVD events) found ALA intake associated with a modest ~10-14% lower CVD risk in dietary-intake studies (pooled RR 0.9, 95% CI 0.81-0.99) — but the more rigorous biomarker-based studies showed a similar-direction trend that was not statistically significant (pooled RR 0.8, 95% CI 0.63-1.03). This is observational evidence, not RCT hard-outcome evidence — there is no ALA equivalent of VITAL, ASCEND, REDUCE-IT, or GISSI-Prevenzione.
Fourth, a modest lipid/BP RCT signal exists and is real. Paschos 2007 found 8g ALA/day lowered blood pressure at 12 weeks (p=0.016 systolic, p=0.011 diastolic); Kawakami 2015 found flaxseed oil lowered small dense LDL, a more atherogenic LDL subfraction.
