Its best-supported use is post-surgical (dental) swelling — a 6-RCT meta-analysis found real swelling and late-pain reduction, but no effect on trismus. Its second real thread is a modest, thin adjunct signal for chronic sinusitis. What bromelain is, and the two threads that hold up
Bromelain is a mixture of proteolytic (protein-digesting) enzymes extracted from pineapple stems, sold as a supplement for its proposed antiedematous and anti-inflammatory activity. It is not an essential nutrient — there's no deficiency state, no RDA, and no government-set upper limit — so any benefit has to stand on its own trial evidence. Two threads run through the twelve verified trials and reviews in this evidence base. The strongest, and most consistent, is post-surgical swelling, built almost entirely on third-molar (wisdom tooth) extraction RCTs: a 2019 systematic review and meta-analysis of 6 RCTs (Liu et al. 2019) found bromelain significantly reduced facial swelling at both early (P=.03) and late (P=.0004) timepoints and pain at the later day-7 timepoint (P=.002) — but found "no apparent effect on early or late trismus," the limited-jaw-opening measure. Bromelain's proteolytic potency is assayed in activity units — most commonly GDU (Gelatin Digesting Units), also seen as MCU (Milk Clotting Units) or FIP (International Pharmaceutical Federation units, the system used in several German-language trials cited on this site). There is no single public conversion factor that works reliably across brands, because potency is assayed per manufacturing batch. Generally well tolerated across the trials reviewed; mild GI upset is the most commonly reported adverse effect. Four things worth knowing: bromelain has a possible antiplatelet/bleeding-potentiation effect (ex vivo human-blood data, Kaur et al. 2016) and appears on standard herb-warfarin interaction lists — check with a clinician if you take anticoagulants or before any planned surgery; there's a real pineapple/papain/latex allergy cross-reactivity risk; the "boosts antibiotic absorption" claim traces to a 1986 rat study only, never proven in humans; and "bromelain dissolves tumors/cures cancer" is overreach — the underlying cancer research is in-vitro/animal immunomodulation, not a treatment claim.
