Garlic Supplements: The Allicin Most Pills Never Deliver

Garlic modestly supports healthy blood pressure in people who run high — meta-analyses found systolic drops of roughly 8–9 mmHg in hypertensives (Ried 2016), with independent replication (Rohner 2015, who still call the evidence "not strong"). The catch that defines this category: allicin isn't in the clove and is destroyed by stomach acid, so most pills may deliver little active. In one analysis, 83% of enteric brands released <15% of labeled allicin (Lawson 2001). What garlic is (and what actually does the work)
Garlic is marketed for "heart health," and the blood-pressure part is grounded — but the active you're paying for is fragile. In fresh garlic the star compound is allicin, and here's the twist: allicin isn't present in the intact clove. It forms only when garlic is crushed, when a precursor (alliin) meets an enzyme (alliinase). That enzyme is destroyed by stomach acid — so a pill has to actively protect the reaction to deliver anything at all (Lawson 2001). Two forms manage it: enteric-coated or stabilized allicin (built to survive the stomach) and aged garlic extract (AGE), whose active is the stable compound S-allylcysteine and which carries most of the blood-pressure trial data. That single fact — the active is allicin (or AGE's S-allylcysteine), not the "equivalent mg" — is why the big number on most bottles tells you almost nothing.
Does it work? (the short version)
Modestly, and mostly for blood pressure in people who already run high. Meta-analyses by Ried and colleagues found garlic lowered systolic blood pressure by roughly 8 mmHg in hypertensives (Ried 2008; Ried 2016; Ried 2020), and an independent group replicated a similar effect (about 9 mmHg) — while explicitly cautioning the evidence is "not strong" because trials are heterogeneous (Rohner 2015). The most consistent form is aged garlic extract, tested at 1.2 g/day in a dedicated RCT with no rise in bleeding risk (Ried 2016, AGE-at-Heart). Lipid effects are smaller and conditional (Ried 2013); the cold/immune evidence is insufficient (Lissiman 2014).
The one-line takeaway The evidence is for a delivered active — allicin that survives the stomach, or aged garlic extract — that most products don't provide. Of the 12 we track, only 6 use a real delivery route and 6 print a big "equivalent mg" that isn't allicin — so shop on delivery route first, not on the milligram number.
The buying problem: delivery, not "equivalent mg"
Two traps. First, the label: "1000 mg equivalent" and "2400 mg equivalent" describe fruit-equivalent or powder weight, not the allicin that reaches your blood (the delivery problem). Second, the comparison: because plain powders and garlic-oil softgels can't reliably deliver an active, a bigger cheap number isn't a better buy (best garlic by delivery). The honest move is to prioritize a product whose form actually delivers — the 6 that do (enteric/stabilized allicin, or aged garlic extract) are the only ones where the dose means anything.
