How Long Does Tongkat Ali Take to Work?

By Erin Rose · Published · Methodology · About Us

Informational summary of published clinical trials — not medical advice. Tongkat ali root can accumulate heavy metals; buy only third-party-tested products, and avoid it if you have a hormone-sensitive condition or are pregnant.

Tongkat ali's shortest trial ran just 4 weeks and found lower cortisol and higher testosterone in stressed adults. A separate trial in men with low testosterone measured its result at 1 month. The longest trial, in men with age-related hormone decline, ran 6 months.

There's no single answer. Durations range from 4 weeks to 6 months depending on the outcome, and this hub's own pages don't fully agree on how long to wait before judging it either.

Timeline by Goal

Each row below is a trial's actual duration, not a growth curve. None of these studies checked in earlier and reported a faster result — the duration listed is when the outcome was measured.

Tongkat ali trial durations as reported in the clinical trials cited on this site
Goal What the Evidence Shows Source-Trial Duration
Stress-related cortisol & testosterone (Talbott 2013) Cortisol down about 16%, testosterone up about 37% vs placebo, in 63 moderately stressed adults on 200mg/day Measured at trial's end; trial ran 4 weeks total
Low-normal testosterone (Tambi 2012) 90.8% of 76 hypogonadal men reached normal testosterone values on 200mg/day Measured at trial's end; trial ran 1 month total
Age-related hormone decline / ADAM (Leitão 2021) Improved erectile function and testosterone in 45 men on 200mg/day, placebo-controlled Measured at trial's end; trial ran 6 months total — the longest cited here

A 6-month trial doesn't mean tongkat ali is 'slower' for aging men — it means that's the outcome researchers chose to measure over 6 months. Why the Hub Gives Two Different Reassessment Windows

This hub's own pages disagree on how long to wait. The dosage guide says give it 2-4 weeks. The testosterone deep-dive says changes build over 4-12 weeks and suggests reassessing at 2-3 months.

We're not going to silently pick one. If you're taking tongkat ali for a general sense of whether it's doing anything, 2-4 weeks is a reasonable first check. If testosterone specifically is your goal, use the longer window — 2-3 months — because that's what the page built specifically around the testosterone trials recommends, and it lines up with the 1-month and 6-month trial endpoints in the table above.

What Changes the Timeline

  • Which outcome you're tracking. Stress-related cortisol and testosterone moved in 4 weeks. Restoring low testosterone toward normal took 1 month. Erectile function and testosterone in aging men were measured at 6 months. These are different questions with different answers, not one supplement working at different speeds.
  • Starting point. All three trials above studied men who started low — stressed, hypogonadal, or aging. A trial in young elite athletes with normal testosterone found no effect at all, at any duration (Zakaria 2023, PMID 37398958).
  • Dose consistency. All three trials in the table used 200mg/day of standardized extract. None tested whether a higher or lower dose changes the timeline.

Frequently Asked Questions

Is there one timeline for how long tongkat ali takes to work?

No single number. The trials behind this site range from 4 weeks to 6 months, and each measured a different outcome. This hub even gives two different reassessment windows: 2-4 weeks in the dosage guide, and 2-3 months in the testosterone deep-dive. Use the longer window if testosterone is your goal.

How long until tongkat ali helps with stress-related testosterone?

Give it 4 weeks. That is how long the one trial that measured this ran. In 63 moderately stressed adults, 200mg/day for 4 weeks dropped cortisol about 16% and raised testosterone about 37% versus placebo (Talbott 2013, PMID 23705671). The trial did not report an earlier check-in.

How long for tongkat ali to help low testosterone specifically?

One month, in the trial that tested it. In 76 men with late-onset hypogonadism, 200mg/day for 1 month brought 90.8% of patients to normal testosterone values (Tambi 2012, PMID 21671978). That is a 1-month endpoint, not an earlier detection point.

What was the longest tongkat ali trial, and what did it show?

Six months. A placebo-controlled RCT in 45 men with age-related hormone decline (ADAM) gave 200mg/day for 6 months and found improved erectile function and testosterone (Leitão 2021, PMID 33541567). It is the longest human trial cited on this site.
