Does Tongkat Ali Actually Work? What the Evidence Shows

Tongkat ali's testosterone evidence is real, but conditional. A 2022 meta-analysis found it significantly raised testosterone in men who start low, not in men with normal levels. It's not a steroid, and it won't push normal testosterone higher.

Separately, a stress trial found it lowered cortisol and raised testosterone in stressed adults over just 4 weeks. The evidence gets weaker for young, healthy men.

Given the two real quality problems above — heavy-metal contamination and vague "extract ratio" labels — only two kinds of products are worth recommending: ones that disclose a specific eurycomanone percentage and back it with independent lab testing.

Testosterone: real, but only if you start low

The strongest single data point is a 2022 systematic review and meta-analysis that pooled human trials. It found tongkat ali significantly improved serum total testosterone in men (Leisegang 2022, PMID: 36013514). But the effect isn't uniform.

The increase showed up specifically in men with low testosterone. It wasn't significant in men with normal levels.

A six-month double-blind, placebo-controlled RCT supported benefits on testosterone-related measures in men with androgen deficiency of aging (Leitão 2021, PMID: 33541567). The pattern holds across the evidence: tongkat ali helps restore suppressed testosterone toward normal. It doesn't push already-normal testosterone higher.

If you're a healthy young man with normal levels, don't expect a dramatic change. In one trial of young elite athletes, it produced no effect at all (Zakaria 2023, PMID: 37398958).

How it works — and why stress matters

There are two proposed direct mechanisms. Eurycomanone appears to inhibit aromatase, the enzyme that converts testosterone into estrogen, so less testosterone gets lost to conversion. It may also reduce binding to sex hormone-binding globulin (SHBG), freeing up more bioavailable testosterone.

But one of the more striking data points comes from a different angle. A randomized, double-blind, placebo-controlled trial followed 63 moderately stressed adults for 4 weeks on 200mg/day. Cortisol dropped about 16% and testosterone rose about 37% versus placebo (Talbott 2013, PMID: 23705671).

A 37% testosterone increase sounds dramatic. But it started from stressed people with suppressed testosterone. The real mechanism may be cortisol reduction letting testosterone normalize, not a direct androgenic push.

That's also why the clearest benefits cluster in older, stressed, or low-T men rather than healthy young men.

Libido and strength: partly independent of the testosterone number

Not everything tracks the testosterone data cleanly. One randomized trial found improved libido and erectile-function scores even without a significant testosterone change. That suggests tongkat ali may work through additional pathways beyond raw hormone levels (Ismail 2012, PMID: 23243445).

For strength and muscle, the picture is genuinely mixed. One trial found some benefit in older adults (Henkel 2014, PMID: 23754792). A separate trial found no effect in young elite athletes (Zakaria 2023, PMID: 37398958).

It's the same pattern as the testosterone evidence: helps those who start behind, does little for those who don't.

The quality problems this category has to be honest about

Even where the biology checks out, tongkat ali has two documented problems that have nothing to do with whether the herb "works":

  • Heavy-metal contamination. Tongkat ali root can accumulate heavy metals from soil. Surveys of commercial products have repeatedly found real problems. One analysis found about 36% of products exceeded the mercury limit (Ang 2004, PMID: 15162849). A separate survey found about 26% noncompliant (Ang 2006, PMID: 16567029). Buy only third-party-tested products.
  • Undisclosed eurycomanone content. Eurycomanone is the marker compound the positive trials standardized to. A label that only states a "200:1" or "100:1" extract ratio describes how much root was used to make the extract. It doesn't tell you how much active eurycomanone ended up in it, so the ratio can hide a weak product. A disclosed eurycomanone percentage (2%, 10%) is the meaningful quality marker — compare products on that, not on raw milligrams or extract ratios.

The same honesty applies to the broader "test booster" aisle. Fadogia agrestis, often sold alongside tongkat ali, has zero human clinical trials — only rat studies, including one showing testicular toxicity.

What tongkat ali does not do

  • It will not replace TRT. If you have clinically diagnosed hypogonadism, this is not a substitute for medical treatment.
  • It will not raise testosterone above your natural ceiling. It is not an anabolic steroid.
  • It will not do much if your testosterone is already normal. The clearest evidence is in men starting low or under stress.
  • It is not automatically a quality product just because it's labeled "tongkat ali." Contamination and undisclosed content are real, documented risks — standardization and third-party testing are not optional extras.
