Spirulina Side Effects: GI Upset & Who Should Avoid It

Informational summary of published research — not medical advice. Spirulina stimulates the immune system and has real cautions for autoimmune disease, phenylketonuria, and certain medications — see below before starting. Data on use in pregnancy and breastfeeding is lacking.

Properly cultivated spirulina (Arthrospira platensis) is generally well tolerated. What gets called a spirulina "side effect" is really two very different things. The direct effects are mild and short-lived — stomach upset, gas, and headache. The scarier stuff — toxin exposure — is almost always a product problem: a poorly sourced or contaminated batch, or a different wild-harvested organism sold under a similar "blue-green algae" label, not a property of cultivated spirulina itself. The genuine contraindications are narrower and worth taking seriously: spirulina is a high-protein food that matters for phenylketonuria, it's an immune stimulant that can be risky with autoimmune disease or immunosuppressant drugs, and it has theoretical additive effects with anticoagulants and antidiabetic medication.

Two different problems get called “side effects”

It's worth separating these before anything else, because they have almost nothing to do with each other. One is a mild, direct physiological response to the alga — the kind of thing you'd get from any new dense whole-food supplement. The other is contamination: heavy metals or microcystin toxins that end up in a product because of how (or where) it was grown or processed, or because it wasn't spirulina at all but a wild-harvested lookalike. Briefly: when researchers screened algal supplements for microcystin, cultivated spirulina tested negative while wild-harvested AFA (Aphanizomenon flos-aquae, a different organism sold as "blue-green algae") tested positive up to 5.2 µg/g (Vichi 2012). So a toxin exposure risk is a reason to check a product's source and testing — it is not a property of spirulina you should expect to run into.

The direct effects: mild GI upset and headache

The side effects actually attributable to spirulina itself are minor and short-lived. The NIH's drug and lactation reference lists diarrhea, bloating, upset stomach, flatulence, edema, headache, muscle pain, facial flushing, and sweating as spirulina's minor side effects (LactMed, PMID: 30000909). None of the clinical trials behind spirulina's cardiometabolic evidence — on lipids, blood pressure, or fasting glucose — reported a safety signal beyond this kind of mild tolerability issue. If you notice GI discomfort, it's reasonable to start at a lower amount within the studied 1–8 g/day range and build up, the same approach used for any new whole-food supplement, though there isn't a published dose-response study specifically for these effects to confirm that helps.

Autoimmune disease and immunosuppressant medication: the real caution

Spirulina is a genuine immune stimulant — that's the flip side of its marketed "immune support" benefit, and it's the caution that matters most on this page. A 2025 case series reported autoimmune disease onset or flares, including dermatomyositis, temporally associated with spirulina use (de Carvalho 2025). Separately, an in vitro laboratory study exposed immune cells drawn from dermatomyositis patients to spirulina and found it triggered inflammatory cytokine production (interferon-β, TNF-α, interferon-γ) primarily through Toll-like receptor 4 (TLR4) activation — a plausible mechanism, tested on patient-derived cells in a lab, not proof that spirulina causes autoimmune disease in a general population (Bax 2023).

If you have an autoimmune disease, a family history of one, or take immunosuppressant medication, talk to your doctor before starting spirulina. The same immune-activating action people take spirulina for is the plausible reason it's been linked to reported flares, and stimulating the immune system runs directly against the point of an immunosuppressant drug. Phenylketonuria: it's a protein food, and protein means phenylalanine

Spirulina is not a standardized extract — it's a whole dried biomass that runs up to 70% protein by weight (Karkos 2011). Any food that dense in protein contributes meaningful phenylalanine, the amino acid restricted in phenylketonuria (PKU). We haven't found a study measuring spirulina's phenylalanine content specifically, so we won't invent a number here — but the underlying nutrition logic is not in question: if you or a family member manages PKU, a scoop of spirulina needs to be counted toward the daily phenylalanine allowance the same way any other high-protein food would, in coordination with the dietitian managing that diet, not treated as a "natural" exception.

Anticoagulants and antidiabetic drugs: theoretical additive effects

Two more interactions are plausible on mechanism but haven't been tested as an actual drug-supplement combination in people taking spirulina — which is a real limitation, not a reason to dismiss them.

  • Antidiabetic medication. Spirulina measurably lowers fasting blood glucose — about 18 mg/dL across a meta-analysis of type 2 diabetes trials, with no significant change in HbA1c (Hatami 2021). Adding that effect on top of insulin or an oral antidiabetic drug could push blood sugar lower than intended. If you're on either, monitor your glucose and mention spirulina to your doctor.
  • Anticoagulants. Calcium spirulan, a sulfated polysaccharide isolated from Spirulina platensis, increased the antithrombin activity of heparin cofactor II more than 10,000-fold in vitro (Hayakawa 1996). That's a laboratory finding on an isolated compound, not a demonstrated effect of taking a spirulina supplement — but it's specific and mechanistically real enough that if you take a blood thinner, it's worth telling your doctor you're adding spirulina.
