Which L-Carnitine? LCLT vs ALCAR vs PLC, Matched to Your Goal

The short answer
Four forms, four different jobs — but they're all sold as generic "energy / fat metabolism." LCLT is exercise recovery (Stefan 2021). ALCAR crosses the blood-brain barrier — the brain & nerve form (Montgomery 2003, small; Li 2015). PLC is circulation / claudication and is nearly unbuyable (Kamoen 2021). Plain is the generic base. And the label traps are real: two products sold as "L-Carnitine" are actually LCLT, and standalone PLC barely exists.

LCLT — the exercise form
L-carnitine L-tartrate is base L-carnitine paired with tartaric acid as a salt; the tartrate helps absorption, but the reason it matters is that the exercise-recovery evidence was built on this form. A 2021 trial found five weeks of LCLT improved recovery and reduced markers of muscle damage such as creatine kinase (Stefan 2021). If your goal is training recovery, this is the form to match — not ALCAR, not PLC.
ALCAR — the only form that reaches the brain
Acetyl-L-carnitine carries an acetyl group, and that group lets it cross the blood-brain barrier. That's the entire reason ALCAR — and not LCLT — is the form studied for the brain and nerves. Be honest about the size of each signal: the cognitive benefit in mild cognitive impairment / mild Alzheimer's is small, and a review has questioned how those trials were pooled, so hold it loosely (Montgomery 2003). The neuropathic-pain evidence is more consistent, with a larger effect in people with diabetes (Li 2015). It's the most crowded shelf of the four — 6 of the 14 products we track are ALCAR.
PLC — the circulation form you can barely buy
Propionyl-L-carnitine is the form with the vascular evidence: a Cochrane review found it increased pain-free walking distance in intermittent claudication by about 26% versus placebo (Kamoen 2021). The practical catch is availability. In our tracked set there's only one standalone PLC product, from a small brand, plus one multi-form blend that folds in a partial PLC dose. The big brands that stock LCLT and ALCAR mostly don't sell PLC — so the person who actually has the matching condition is the one who can't find the matching form.
The other way to get any PLC is a blend: Life Extension Optimized Carnitine, three forms incl. 300 mg PLC (60 Capsules) combines ALCAR, ALCAR-arginate, and a partial (~300 mg) PLC dose — useful for coverage, but you can't attribute a full claudication dose to it.
Plain L-carnitine — the generic base
Plain L-carnitine (free-form, fumarate, or liquid) is the unmodified molecule with no special targeting. It shows up in general and heart-related research and is a reasonable, cheap base if you don't need one of the specialized forms — but it doesn't carry LCLT's exercise data, ALCAR's brain access, or PLC's vascular evidence. Put together Pick the form that matches your goal — LCLT for exercise, ALCAR for brain/nerve, PLC for circulation — then compare cost per gram within that form. Buying the cheapest carnitine outright usually lands you on LCLT, which is only the right buy if exercise recovery is what you're after.

What to actually do
- Start from the goal, not the milligrams. Exercise → LCLT. Brain/nerve → ALCAR. Circulation → PLC. General/base → plain.
- Read the chemical name on the panel — "L-carnitine L-tartrate," "acetyl-L-carnitine," "propionyl-L-carnitine" — not the big front word.
- Don't overpay for a name. Once you've fixed the form, compare cost per gram within it.
- If you want the circulation form, plan ahead — standalone PLC is scarce, and blends only give a partial dose.
