Do EAAs Help Older Adults Build or Keep Muscle? The Trial Evidence

The honest answer
Older adults are the single most defensible EAA use case on this site — and even here, the evidence is genuinely split, not a clean win. Real acute signal exists: a 15g EAA bolus raises muscle protein synthesis in elderly subjects same as in young ones (Paddon-Jones 2004), and a leucine-enriched formula can restore a blunted response in elderly subjects that a standard formula can't (Katsanos 2006). But two honest counterweights matter just as much: healthy older adults may not be "anabolic resistant" at all (Moro 2018), and the one real 12-week strength/size training study found no added benefit from amino-acid supplementation over training alone (Godard 2002).
The positive case: EAAs stimulate muscle protein synthesis in elderly subjects
Paddon-Jones et al. 2004 gave a 15g EAA bolus to 6 young (34±4y) and 7 elderly (67±2y) subjects using stable-isotope infusion and muscle biopsies. The primary endpoint — mixed-muscle fractional synthetic rate — was met in both groups: synthesis rose by about 0.04%/h after ingestion in young and elderly alike. The kinetics differed (slower but more sustained plasma amino acid rise in elderly subjects, and a much smaller insulin response), but the net synthesis outcome was similar once corrected for muscle intracellular retention. The authors' conclusion: "amino acid supplementation acutely stimulated muscle protein synthesis in both young and elderly individuals." This is real, primary-endpoint data — but a very small sample (6/7), a single acute bolus, and no comparison to whole protein or whey.
The leucine wrinkle: older adults may need more of it
Katsanos et al. 2006 compared a 6.7g EAA dose at a whey-like 26% leucine against a leucine-enriched 41% version. In elderly subjects, the fractional synthetic rate did NOT increase with the 26% formula (basal 0.044%/h → 0.049%/h, not significant) but DID increase with the 41% formula (0.038%/h → 0.056%/h, P<0.05). In young subjects, both formulas worked. The authors' stated conclusion: "increasing the proportion of leucine in a mixture of EAA can reverse an attenuated response of muscle protein synthesis in elderly but does not result in further stimulation of muscle protein synthesis in young subjects." This is the clearest mechanistic evidence in this pack that standard, whey-ratio EAA formulas may under-deliver for some older adults, and that a leucine-weighted formula is a real, age-specific tool — not marketing gloss.
The honest counterweight: healthy older adults may not be "anabolic resistant" at all
Moro et al. 2018 followed 19 healthy older adults (71±4y) through 12 weeks of progressive resistance-exercise training, measuring muscle protein synthesis and mTORC1 signaling in response to a 6.8g EAA dose before and after training. Training itself worked as expected — strength up 16%, lean mass up 2%, muscle cross-sectional area up 27% (all significant). But the magnitude of the MPS/signaling response TO EAA ingestion didn't change with training, and importantly, these healthy, community-dwelling older adults showed a normal EAA-stimulated synthesis response throughout. The authors' conclusion, stated plainly: "anabolic resistance to amino acids may not be a problem in healthy older adults." Read against Paddon-Jones and Katsanos, this doesn't contradict their findings so much as narrow who the "anabolic resistance" pitch actually applies to — likely frailer, sarcopenic, or clinically compromised seniors, not healthy independent older adults in general.
The counterweight that matters most: no added strength or size over 12 weeks
Godard, Williamson & Trappe 2002 ran the real functional-outcome test this evidence base is missing elsewhere: 17 older men (>65y) completed 12 weeks of progressive knee-extensor resistance training, with one group (n=8) also receiving a daily oral 8-amino-acid complex (note: this formula excluded tryptophan, so it is not a complete 9-EAA product) combined with carbohydrate, and a control group (n=9) receiving no supplement. Isometric strength, 1RM strength, and thigh cross-sectional area (by CT scan) all improved significantly WITH TRAINING in both groups — isometric strength up 21%, 1RM up 50%, thigh CSA up 6.5% — but with no significant difference between the amino-acid and control groups on any measure. The authors' conclusion: "whole muscle strength and size are not enhanced with a postexercise daily provision of an oral amino-acid complex during 12 wk of PRT in older men." This is the direct counterweight to the acute-signal trials above: a real, weeks-long functional outcome found the supplement added nothing over training and normal eating.
Where the honest niche actually is
A 2021 narrative review (Ispoglou et al., not primary data) frames EAA supplementation as a strategy to raise effective protein intake specifically for older adults who are struggling to hit protein targets through food — non-satiating, layerable onto a protein-light meal, with palatability and long-term compliance flagged as the real-world bottleneck, not efficacy. Read alongside the trial evidence above, the most defensible picture is: EAAs can plug a real, acute protein gap for older adults who aren't eating enough, particularly with a leucine-enriched formula; but there's no trial in this evidence base showing that translates into a strength or muscle-mass advantage for someone already training and eating adequately, and Godard's 12-week data argues the opposite. This is not a treatment for sarcopenia or any diagnosed muscle-wasting condition — it's a food-gap tool, at best, backed by acute signaling data more than functional outcomes.
