R vs Racemic Alpha-Lipoic Acid: Only R Is Active (But Racemic Is Cheaper)

The short answer

Alpha-lipoic acid has two mirror-image forms, and only R-(+) is the body-made, active one (Salehi 2019). Most products are racemic (50/50 R/S), so a "600 mg" racemic cap = ~300 mg active R. The obvious conclusion — "buy R-ALA" — is where the naive story goes wrong. Priced per mg of actual active R, bulk racemic is the cheapest source ($0.03/100 mg R); R-ALA / stabilized Na-R-ALA cost ~6× more per mg of R. R-ALA's genuine edge is absorption, not cost (Carlson 2007). And watch the "R-ALA Complex" trap — a name that sounds like R-ALA but is mostly racemic.

Axis 1: only R-(+) is active, so half a racemic dose is "wasted"

Alpha-lipoic acid is a chiral molecule: it exists as two non-superimposable mirror images, R-(+) and S-(−). Only the R-(+) enantiomer is the form the body synthesizes and the biologically active one; the S form is a manufacturing by-product with far weaker activity (Salehi 2019). Making plain ALA yields a racemic 50/50 mix, so a labeled "600 mg alpha-lipoic acid" delivers about 300 mg of active R and 300 mg of the near-inactive S. This is the fact that makes people reach for R-ALA — and on its own it's true. The mistake is jumping from "only R is active" straight to "R-ALA is the better buy."

Axis 2 — the honest twist: racemic is still the cheapest source of R

Here's where the naive story breaks. If you compute cost per milligram of the actual active R across products, the ranking inverts. Bulk racemic is the cheapest source of active R — because it's manufactured at scale and sold in large counts, its active-R cost is the lowest of the field even after you throw away the inactive S half. Premium R-ALA and stabilized sodium-R-lipoate cost roughly 6× more per mg of R. So paying up for R-ALA does not buy you more active per dollar.

So why pay for R-ALA at all? Absorption, not cost

R-ALA has one genuine, honest advantage, and it's pharmacokinetic. Stabilized sodium-R-lipoate (Na-R-ALA) reaches higher peak plasma levels (Cmax) and total exposure (AUC) than plain racemic ALA in human testing (Carlson 2007). So if better absorption of R matters to you, a stabilized Na-R-ALA product is a reasonable choice — you're buying bioavailability, not a cheaper or larger active dose.

The label trap: "R-Alpha Lipoic Acid Complex" that isn't

The sharpest trick is a name that sounds like pure R-ALA but is mostly ordinary racemic. A product marketed as an "R-Alpha Lipoic Acid Complex" can be largely racemic ALA with only a small added R fraction — so you pay an R-ALA-sounding price for a largely racemic dose. Read the Supplement Facts panel for the actual R milligrams, not the front-of-bottle name.
