NMN is marketed on longevity claims that rest almost entirely on mouse data. The human evidence is smaller, shorter, and more mixed than the marketing suggests. What it reliably shows: NMN raises blood NAD+ — a biomarker, not a proven health outcome — at every dose tested. Beyond that, functional results are individually small and inconsistently replicated: a walking-speed and aerobic-capacity signal shows up across several small trials, one trial found improved insulin sensitivity in prediabetic women but carries a published critique, and the single most rigorous piece of evidence — a 2024 meta-analysis pooling 8 randomized trials — found no significant benefit on glucose control or lipid profile. No human trial has tested lifespan or anti-aging outcomes at any dose. NMN is sold on mouse data — the human trials tested something narrower

Most NMN marketing leans on mouse studies: extended lifespan, reversed markers of cellular aging, restored NAD+ to youthful levels. That research is real, but none of it transfers automatically to a human longevity claim. The human evidence base for NMN is eight verified randomized controlled trials, and every one of them ran 12 weeks or less, measuring short-term biomarkers or functional endpoints — blood NAD+ levels, walking distance, insulin-clamp glucose disposal, sleep-quality questionnaires — never lifespan, aging biomarkers, or disease outcomes. That gap between what's marketed and what's actually been tested in humans is the single most important thing to understand before reading any specific benefit below.

The one reliable finding: blood NAD+ goes up. That's a biomarker, not an outcome

NAD+ (nicotinamide adenine dinucleotide) is a coenzyme involved in energy metabolism and cellular repair that declines with age. Raising it is NMN's whole mechanistic premise, and on this specific point the human data is genuinely consistent: blood NAD+ and its metabolites rise after NMN supplementation in every trial that measured it, across doses from 250 to 1200mg/day. Two examples: a 6-12 week trial in healthy older men found blood NAD+ rose significantly on 250mg/day, alongside nominal gait-speed and grip-strength improvements, though it was funded by Mitsubishi Corporation and half the randomized participants didn't complete the study (Igarashi 2022, PMID: 35927255). A separate 12-week trial in older adults also found blood NAD+ levels increased significantly (Morifuji 2024, PMID: 38789831). The honest caveat: a rising blood marker confirms NMN is doing what it's pharmacologically designed to do. It is not, by itself, proof of a clinical benefit — the sections below are where that gets tested directly, and the results are far less consistent.

Walking speed and aerobic capacity: the best functional evidence — still small and short

This is NMN's strongest functional pocket, and also the one most worth reading precisely. Recreationally-trained runners taking 300, 600, or 1200mg/day for 6 weeks showed dose-dependent improvements in oxygen uptake at the ventilatory thresholds at the 600 and 1200mg doses. VO2max itself — peak oxygen uptake, the headline measure of aerobic capacity — did not change, and neither did peak power (Liao 2021, PMID: 34238308). Healthy middle-aged adults taking 300, 600, or 900mg/day for 60 days improved 6-minute walk distance at every dose, with 600-900mg producing the longest distances (Yi 2023, PMID: 36482258). But two of the five trials that measured a walking or physical-function outcome had null primary endpoints, with only secondary or subgroup results turning up positive — that distinction matters, because leading with the secondary finding overstates what the trial actually showed. Insulin sensitivity and blood sugar: one headline trial, one meta-analysis that found nothing

This is the benefit most likely to be oversold. The most-cited positive NMN study in the popular press is Yoshino 2021 (PMID: 33888596), an NIH-funded trial published in Science that found improved muscle insulin sensitivity in a small group of prediabetic postmenopausal women. It also carries a published critique: the NMN group started with significantly lower baseline liver fat than the placebo group (6.3% vs 14.8%, p=0.003) — a randomization imbalance directly relevant to an insulin-sensitivity outcome, which undercuts how much of the result can be credited to NMN itself. Weighing far more heavily against a blood-sugar benefit is Chen 2024 (PMID: 39531138), a systematic review and meta-analysis pooling 8 randomized controlled trials across doses from 250 to 2000mg/day, which found NMN supplementation did not show a significant effect on glucose control or lipid profile — the authors state plainly that the evidence "do[es] not support the use of NMN supplementation among the general population to improve glucose and lipid metabolism." That meta-analysis pools more data than any single trial in this evidence base, and it's a null result. It belongs in the center of this conversation, not a footnote.

NMN vs. NR, and the regulatory story worth knowing before you buy

NMN and NR (nicotinamide riboside) are both NAD+ precursors sold on similar claims, and the honest comparison isn't about which one "works better" — neither has a proven longevity or disease-outcome benefit in humans. Where they differ concretely is regulatory history. In November 2022 the FDA took the position that NMN is excluded from the legal dietary-supplement definition, because it had been authorized for investigation as a new drug before being marketed as a supplement ingredient — a technical drug-preclusion trigger, not a safety finding against NMN. Amazon pulled NMN listings by March 2023. After a citizen petition and a 2024 lawsuit from the Natural Products Association, the FDA reversed course in two letters dated September 29, 2025, confirming NMN is not excluded, and Amazon relisted it within weeks. NR never went through any of that — it has kept an uninterrupted regulatory standing throughout. That whiplash doesn't tell you anything about whether NMN works, but it's a real reason to buy from a brand that discloses third-party testing rather than lean on brand-name trust alone.
