Why per milligram, and what the trials found about dose

Across 19 placebo-controlled trials in 1,683 people, melatonin shortened time to fall asleep by about 7 minutes and lengthened total sleep by about 8 minutes, with a small improvement in sleep quality; longer trials and higher doses showed larger effects on latency and total sleep, but dose made no difference to sleep quality (Ferracioli-Oda 2013, PMID 23691095). The reason the per-milligram figure is nominal is measured: in 31 supplements, content ranged from 83% below to 478% above the label, the same product varied by up to 465% between lots, and 71% missed their claim by more than 10% (Erland 2017, PMID 27855744); a 2023 JAMA analysis of melatonin gummies reported the same labelled-versus-measured pattern (Cohen 2023, PMID 37097362). Price the milligram, but buy the product whose milligram has been checked.

Reading the quality column
