That's arginine's strongest single piece of evidence — but it was measured in a general population, and it does not extend to people who've recently had a heart attack, where a landmark trial found the opposite: higher mortality (Schulman 2006, VINTAGE MI).

Dong 2011 pooled 11 randomized, double-blind, placebo-controlled trials totaling 387 participants, with oral arginine doses spanning 4–24 g/day. The topline result: systolic blood pressure fell by 5.39 mmHg (95% CI −8.54 to −2.25, P=.001) and diastolic by 2.66 mmHg (95% CI −3.77 to −1.54, P<.001) compared with placebo. Two details make this a more credible finding than a single small trial: the effect held up in a sensitivity analysis restricted to trials lasting 4 or more weeks, and separately in a sensitivity analysis restricted to participants not on antihypertensive medication — meaning the result isn't just an artifact of short trials or drug interactions skewing the pooled number.

Why this is a sustained-use effect, not a single-dose one
It matters that Dong 2011's benefit specifically held up in trials lasting 4+ weeks, because a single acute dose does not appear to move blood pressure the same way. Bode-Böger 1998 gave 8 healthy men a single 6 g oral dose and found no significant blood-pressure change; only a much larger 30 g IV infusion produced an acute effect (BP −4.4%, total peripheral resistance −10.4%). ⚠⚠ The blood-pressure benefit above does NOT apply to post-heart-attack patients — the opposite was found. The VINTAGE MI randomized trial (Schulman 2006, JAMA) gave 153 patients L-arginine (goal 9 g/day) or placebo for 6 months starting after a first heart attack. It found no improvement in vascular stiffness or ejection fraction, and 6 participants (8.6%) in the arginine group died during the study versus none in the placebo group (P=.01) — enrollment was stopped early for safety. The authors' own conclusion: "L-arginine...may be associated with higher postinfarction mortality. L-arginine should not be recommended following acute myocardial infarction." Dong 2011's healthy-population BP benefit is a completely separate finding from this cardiac-population harm signal — do not extrapolate one to the other. Anyone with a recent heart attack or unstable cardiac disease should not take L-arginine for blood pressure or any reason without a clinician's direct guidance.
Interaction caution
Because arginine's own effect on blood pressure is real, it can be additive with prescribed antihypertensive medication, raising the risk of symptomatic low blood pressure. The same caution applies to nitrate medications and PDE5 inhibitors (sildenafil/tadalafil-class drugs), where combined vasodilation could be more pronounced.
