L-Arginine vs. L-Citrulline: Which One Actually Raises Nitric Oxide?

Arginine is the direct chemical precursor to nitric oxide — but swallowed as a supplement, a head-to-head crossover trial found L-citrulline raises blood arginine levels MORE effectively than arginine itself. The reason is first-pass metabolism: oral L-arginine is heavily broken down by gut and liver enzymes before it reaches circulation (only ~68% bioavailable on average, 51–87% range), while L-citrulline dodges that bottleneck entirely and gets converted to arginine downstream in the kidneys (Schwedhelm 2008). Gram for gram, citrulline is the better-supported oral nitric-oxide precursor — arginine's own strongest evidence is a separate, modest blood-pressure effect.

The trial: 20 volunteers, 6 dosing regimens, one clean head-to-head answer

Schwedhelm 2008 ran a double-blind, randomized, placebo-controlled crossover trial in 20 healthy volunteers, testing 6 different dosing regimens of placebo, citrulline, and arginine, each for 1 week. The primary endpoint — plasma L-arginine pharmacokinetics (peak concentration and total exposure over time) — was met decisively in citrulline's favor. Quoted directly from the trial: "L-Citrulline dose-dependently increased AUC and C(max) of plasma L-arginine concentration more effectively than L-arginine (P<0.01)." At the highest citrulline dose tested (3 g twice daily), the arginine/ADMA ratio — a marker of how much "usable" arginine is available relative to a natural nitric-oxide-synthase inhibitor — rose from 186±8 to 278±14 (P<0.01), and both urinary nitrate and cGMP increased significantly (both P≤0.04). These are real nitric-oxide-pathway biomarkers, not just a blood-level number.

Limits, honestly: healthy volunteers only, not a clinical-outcome trial; FMD itself was null for both arginine and citrulline; small sample (n=20). The result establishes a pharmacokinetic/biomarker advantage for citrulline, not a proven downstream clinical outcome advantage.

Why the "direct" precursor loses: first-pass arginase metabolism

It sounds backwards that citrulline beats arginine at raising arginine. The explanation is first-pass metabolism. Bode-Böger 1998 measured oral L-arginine bioavailability directly: a single 6 g oral dose in 8 healthy men reached the bloodstream at an average of 68%, with individual results ranging 51% to 87%. The missing arginine doesn't vanish — it's broken down by the enzyme arginase, present in high concentrations in the intestinal wall and liver, before the dose ever reaches systemic circulation. L-citrulline is structurally different enough that it isn't a substrate for arginase. It passes through the gut and liver essentially intact, then gets converted to arginine downstream in the kidneys — a route that sidesteps the exact bottleneck that caps oral arginine's effect. That single mechanistic fact is the core reason nitric-oxide-focused supplements have increasingly shifted from arginine to citrulline.

The verdict If the goal is raising blood nitric-oxide precursor levels — pump, circulation, blood-flow support — citrulline is the better-supported choice gram-for-gram. What this doesn't settle

This trial is a pharmacokinetic and biomarker comparison, not a head-to-head clinical-outcome trial pitting arginine against citrulline for blood pressure, exercise performance, or erectile function. Arginine's own strongest clinical evidence — the Dong 2011 blood-pressure meta-analysis (5.39/2.66 mmHg systolic/diastolic reduction at 4–24 g/day) — was generated using arginine itself, not citrulline, so it remains real evidence for arginine specifically, even though citrulline reaches a higher blood arginine level per gram. Don't read "citrulline wins the plasma-level comparison" as "arginine has zero evidence" — the two claims are about different things.

Safety: both arginine and citrulline raise nitric oxide and can lower blood pressure, so use caution combining either with antihypertensive medication, nitrate drugs, or PDE5 inhibitors (sildenafil/tadalafil-class) — effects can add up.
