Why ferritin, not “iron”

Serum ferritin reflects the body’s iron stores and is the first number to fall when intake stops covering losses, months before hemoglobin drops and anemia appears. A kit that measures hemoglobin tells you whether you are already anemic; a kit that measures serum iron tells you what you ate yesterday; ferritin tells you whether the store is empty. The World Health Organization’s 2020 guideline sets 15 µg/L as the ferritin threshold for iron deficiency in apparently healthy adults, and in practice many clinicians treat symptomatic people with a ferritin under 30 (WHO 2020). The catch is that ferritin is also an acute-phase protein: infection, obesity and chronic inflammation push it up and can make an iron-deficient person read “normal,” which is why a clinician pairs it with C-reactive protein; the mail-in kits here report ferritin alone, so a low result is informative and a normal one is not the end of the question. Does a finger-prick blood spot measure ferritin properly?

Yes, and the validation is older than most home-testing companies. Ferritin measured from dried serum spots was compared with the traditional serum assay in 104 people (mean age 24, children included) in Guatemala City: the spot methods correlated at r = 0.71 to 0.88, geometric means did not differ, and the difference between spot and traditional values was under 4 µg/L (Ahluwalia 2005, PMID 15941888). In 100 schoolchildren in Sri Lanka, dried spots from venous and from capillary (finger-prick) blood correlated with traditional serum ferritin at r = 0.88 and 0.86 respectively, and the means were not significantly different (Ahluwalia 2002, PMID 11815320). That is the evidence a mail-in kit rests on: a laboratory measuring ferritin from a dried spot gets the venous answer. The lateral-flow cassettes have nothing comparable; we found no published comparison of any consumer ferritin strip against a laboratory ferritin.

Are rapid ferritin test strips accurate?

Nobody knows, which is the problem. The cassettes on Amazon promise a 15-minute visual result from a finger prick and publish no comparison against a laboratory ferritin; the listings describe lancets and instruction cards, not validation. A lateral-flow test can in principle detect ferritin above a threshold, but a threshold read by eye against a printed image is not the 15 or 30 microgram-per-litre decision you need, and a false "normal" delays the treatment of a common, fixable deficiency.

When should I test my ferritin?

When you have a reason: heavy periods, fatigue or hair loss you cannot explain, a vegetarian or vegan diet, pregnancy planning, endurance training, a bariatric or celiac history, or an iron supplement you are trying to know whether to continue. Ferritin is the earliest marker to fall and the first to recover, so it is also the right number to retest three months into supplementation.
