Hyaluronic Acid Benefits: What Oral Trials Actually Show

Most people know hyaluronic acid as an injection or a skincare serum — the interesting question for a swallowed capsule is whether it does anything at all, since HA is a large polysaccharide that has to survive digestion first. Skin hydration and wrinkle depth is the clearest benefit: four small trials, mostly 8-12 weeks, found modest improvement at 120mg/day, though most of that evidence comes from authors employed by the manufacturer of the ingredient tested. Knee osteoarthritis evidence is thinner and comes from two small trials testing different HA-containing ingredients at different doses (200mg/day plain HA for 12 months vs. roughly 80mg/day of a branded blend for 3 months). Dry eye has only one small trial, and it tested oral HA added on top of topical eye drops, not by itself. None of this is the HA injection some people get into an arthritic knee — that's a different procedure entirely.

Skin hydration and wrinkles: the strongest evidence, and it has its own page
This is the most-replicated finding in the oral-HA category: four small trials, including Oe 2017 (n=60, double-blind, placebo-controlled) and Hsu 2021 (n=40, double-blind, placebo-controlled), found modest improvements in skin hydration, elasticity, and wrinkle depth at 120mg/day over 8-12 weeks. The catch is funding: most of the authors on the two strongest trials are employed by Kewpie Corporation, which also manufactures the Hyabest HA ingredient those trials tested — a caveat worth repeating every time this finding gets cited. Knee osteoarthritis: modest, from two trials that aren't the same intervention
Two small trials test oral HA for knee osteoarthritis, and they used different ingredients at different doses — read them separately, not as one collective "joint dose."
Tashiro 2012, PMID: 23226979 randomized 60 adults with knee osteoarthritis (Kellgren-Lawrence grade 2 or 3) to 200mg/day of plain hyaluronic acid or placebo for 12 months, while both groups also did daily quadriceps-strengthening exercise. Knee symptom scores (the Japanese Knee Osteoarthritis Measure, JKOM) improved over time in both groups; the improvement was described as more pronounced in the HA group, and that difference reached statistical significance against placebo at months 2 and 4 specifically in the subgroup of participants aged 70 or younger. The trial's own abstract does not report the difference as significant for the full 60-person sample, and doesn't report a significance figure at the 12-month endpoint or for the over-70 subgroup. That's a real but narrow finding — an age-subgroup result at two mid-study timepoints, not "oral HA fixes knee osteoarthritis."
Nelson 2015, PMID: 24899570 tested a different, branded ingredient — Oralvisc, a blend of hyaluronic acid with other glycosaminoglycans, at roughly 80mg/day — in 40 obese knee-osteoarthritis patients over 3 months. It found statistically significant improvements in pain and function scores (visual analog scale pain and WOMAC), alongside reductions in several inflammatory markers measured in blood and joint fluid. That's a different ingredient, a different dose, and a quarter of the duration of Tashiro's trial, so it isn't a replication of Tashiro's result — it's a separate positive finding for a different product.
Neither trial tested or has anything to say about an HA injection into the knee (viscosupplementation), a medical procedure a doctor performs directly on the joint. That's a completely different delivery route and evidence base from a capsule you swallow.
Dry eye: one small trial, and it wasn't oral HA alone
The dry-eye evidence is the thinnest of the three, and the trial design matters here: Kim 2019, PMID: 31662894 randomized 54 people with dry eye disease, but every participant used topical hyaluronic acid eye drops throughout the trial — the 24-person study group added an oral HA supplement on top of the drops, while the rest used the drops alone. So this tested oral HA as an add-on to topical treatment, not oral HA by itself against placebo.
In the group that got both, standard dry-eye measures improved over 3 months: the Ocular Surface Disease Index symptom score fell from 61.8 to 42.3 (p<0.001), tear break-up time rose to 4.7 seconds by 3 months (p<0.012), and corneal fluorescein staining — a marker of surface damage — dropped from 1.8 to 0.8 within the first month (p<0.001). The authors concluded the combination worked better than topical HA alone, but the abstract doesn't report the control group's own numbers for a direct side-by-side comparison. Read this as a preliminary signal for oral HA as an add-on to standard drops, not evidence that a capsule treats dry eye on its own.
Does oral HA even reach these tissues?
All three benefit threads above depend on oral HA surviving digestion and reaching skin, joint, or eye tissue in a functional amount — and that mechanism has never been directly tested in humans. Native hyaluronic acid is a large polysaccharide, with molecular weight that can run into the millions of Daltons, well outside the size range most nutrients are absorbed intact. The only evidence it crosses the gut wall and reaches tissue at all comes from two animal tracer studies in rats and dogs (Balogh 2008, Oe 2014) — see the full breakdown of what they found, and didn't establish, on the dedicated absorption page.
