Does Cordyceps Work? The Honest Evidence (and the Safety Landmines)

The honest answer
Cordyceps has modest, mostly small-trial evidence. Endurance: the best trial (Cs-4, 1 g/day, 12 wk) found no VO2max change — only submaximal thresholds rose ~8–10% (Chen 2010). A VO2max gain did show up in Hirsch 2017, but that was a multi-mushroom blend, 4 g/day, n=10 — not cordyceps alone. Immune: NK-cell activity up ~34%, but in small subgroups (Ontawong 2024). There's no proven anti-fatigue effect — call these endurance signals. And the safety cautions are the real headline: immunosuppressants, transplant, autoimmune, bleeding, blood sugar, and myelogenous cancers.
Why "signals," not proof
Cordyceps's plausible actives are beta-glucans and cordycepin, and they only matter if you get the right form — a fruiting body, not grain-grown mycelium. But even with a good product, the human evidence is thin: a handful of small trials, one of which used a blend, and none of which is a rigorous standalone anti-fatigue study. That's why the table below states what each trial actually measured, and where it falls short — overstating cordyceps by stitching these together is the most common way its evidence gets inflated.

Read together: cordyceps shows endurance and immune signals in small trials, but the one trial that isolated cordyceps found no VO2max change, and the positive endurance result came from a blend of 10 people. It's a promising but unproven adjunct — and it only means anything if you get a real beta-glucan dose from the right form, which most products don't disclose.
The safety landmines (the part most pages skip)
These come from Memorial Sloan Kettering's clinical "About Herbs" reference, not randomized trials — they're mechanism- and case-based cautions, but each is a genuine "avoid or clear with your clinician" flag. Because cordyceps stimulates the immune system, several of these follow directly from the same mechanism behind its immune "benefit."
