Colostrum's real benefit is narrower and more specific than the "gut health and immunity" framing selling it. The best-evidenced use is fewer upper-respiratory symptom days in athletes under heavy training — about 44% fewer, from a meta-analysis of 5 small trials. There's real, if inconsistent, evidence that it blunts the spike in intestinal permeability — the specific, lab-measured kind of "leaky gut," not the vague wellness term — after a single hard bout of exercise, though a longer daily-dosing trial found the opposite effect. NSAID-related gut protection has solid animal evidence and one small positive human trial. The mechanistic thread connecting all of this: a real fraction of colostrum's IgG survives digestion intact enough to act locally in your gut, but there's no evidence it gets absorbed into your bloodstream as a systemic antibody boost. General immune-boosting, skin, and hair claims have no dedicated trial evidence behind them at all.

Best evidenced: fewer respiratory symptom days — in athletes specifically

This is colostrum's strongest human data. A systematic review and meta-analysis pooled 5 randomized controlled trials (152 participants), all in people engaged in regular exercise training, followed for 8–12 weeks. Compared with placebo, colostrum significantly reduced the incidence rate of upper respiratory symptom (URS) days by 44% (rate ratio 0.56, 95% CI 0.43–0.72, P<0.001) and URS episodes by 38% (rate ratio 0.62, 95% CI 0.40–0.99, P=0.04) (Jones 2016, PMID: 27462401). That's a meaningful effect size — but the reviewers themselves flagged real limitations: four of the five included trials were judged moderate or high risk of bias due to incomplete reporting, and individual study estimates had low precision. Their own conclusion was that this needs to be followed up with an adequately powered trial before it's settled. And every trial was in exercisers — this doesn't tell you anything about whether a daily scoop reduces colds in a healthy, sedentary adult.

Reducing exercise-induced gut permeability — the measurable kind of "leaky gut"

When researchers say colostrum affects "leaky gut," they mean something specific and testable: the ratio of two sugars, lactulose and rhamnose, that show up in your urine after you drink them. A higher ratio means more of the larger lactulose molecule leaked through your intestinal wall than it should. That's the actual outcome behind these numbers — not the broader, undefined "leaky gut" used in general wellness marketing.

In a double-blind, placebo-controlled crossover trial of 12 volunteers, a single bout of heavy exercise increased that permeability ratio 2.5-fold in the placebo arm (from 0.38 to 0.92, P<0.01). When colostrum was taken for 14 days beforehand, the same exercise bout only pushed the ratio up to 0.49 from the same 0.38 baseline — colostrum truncated the rise by 80% (Marchbank 2011, PMID: 21148400). That's a real, precisely measured effect, from a single acute bout of exercise.

But daily, chronic dosing tells a different story. In a separate trial, 30 men took 60g/day of colostrum, whey protein, or nothing, alongside 8 weeks of running training. After 8 weeks, the colostrum group's permeability ratio had increased by 251%, significantly more than the whey-protein group (+21%, P<0.05) or the no-supplement control group (−7%, P<0.02) — colostrum made permeability worse, not better, under daily dosing during training (Buckley 2009, PMID: 22253980).

A 2024 meta-analysis pooling 10 studies found colostrum produced a significant overall reduction in the lactulose/rhamnose ratio (mean difference −0.24, 95% CI −0.43 to −0.04) — but with extreme heterogeneity between studies (I² = 99%, meaning the trials essentially disagree with each other rather than converge on a consistent effect). The same analysis found no difference between colostrum and control on a separate gut-injury marker, plasma I-FABP (Hajihashemi 2024, PMID: 38361147). The honest read: acute, single-bout gut protection has decent support; daily use during a training block does not, and one well-controlled trial found it backfires.

NSAID-induced gut protection: solid in animals, thinner in humans

This is colostrum's oldest studied use, and it's often cited as human evidence when the strongest data is actually preclinical. In a rat model of NSAID-induced gastric injury, pretreatment with colostrum reduced injury by 30% at a lower dose and 60% at a higher dose, in a clearly dose-dependent pattern; in a parallel mouse model, colostrum in the drinking water prevented the villus shortening that NSAIDs normally cause, while a plain milk preparation did not (Playford 1999, PMID: 10205201). That's animal data, not a human trial.

The human evidence is smaller but real. In a randomized crossover trial, 7 healthy volunteers took the NSAID indomethacin for 5 days; in the control arm, gut permeability rose 3-fold (lactulose/rhamnose ratio 0.36 to 1.17, P<0.01), while no significant increase occurred when colostrum was co-administered. A second arm of the same study tested 15 patients already taking NSAIDs long-term for a medical condition — their baseline permeability was already low despite ongoing NSAID use, and colostrum made no measurable difference in that group (Playford 2001, PMID: 11352778). So the human signal is specifically about starting an NSAID course alongside colostrum, in a small trial — not a demonstrated benefit for people already on long-term NSAID therapy.

The mechanistic crux: does the IgG actually survive digestion?

Colostrum is sold on its IgG content, and the obvious objection is that stomach acid and digestive enzymes should destroy antibody proteins before they do anything. The real answer is more specific than either "yes, it survives" or "no, it's all destroyed."

In a human study, 6 volunteers with an end ileostomy (allowing direct measurement of what reaches the end of the small intestine) took a bovine immunoglobulin concentrate containing 2.1g of IgG. On average, 49% of the ingested IgG was recovered intact in the ileal fluid, rising to 68% in volunteers with faster gut transit times (under 2 hours) versus 36% in slower transit (P<0.05). Critically, the recovered IgG wasn't just structurally intact — its ability to neutralize its target toxin closely tracked how much IgG was recovered (r=0.95, P<0.001), meaning it was still functional (Warny 1999, PMID: 9895380). A broader review of human studies on orally administered immunoglobulin preparations found intact IgG recovered in stool ranges from trace amounts up to 25% of the ingested dose across infants, children, and adults (Jasion & Burnett 2015, PMID: 25880525).

Put together: a real, non-trivial fraction of colostrum's IgG reaches your lower gut intact and functional. That's mechanistically consistent with the local gut-permeability findings above. What it does not show is IgG crossing into your bloodstream — we're not aware of a human study demonstrating that oral colostrum delivers intact antibodies systemically the way an injection or the placenta does in a newborn. Colostrum is naturally packaged in a way that helps protect its bioactive components until they reach the gut, which supports a local mechanism rather than the whole-body "antibody boost" framing in some marketing (Playford & Weiser 2021, PMID: 33477653).

Weak or unsupported: general immunity, skin, hair, and anti-aging

The respiratory-symptom finding above is specific to athletes in heavy training, not a general "boosts your immune system" claim. We are not aware of a controlled human trial testing bovine colostrum supplementation against skin, hair, or anti-aging outcomes at all — a 2021 systematic review of 28 papers on bovine colostrum's health effects found real signal for preventing respiratory illness in athletes, immune modulation, and gut permeability, and conflicting results in pediatric populations, but did not report skin, hair, or anti-aging outcomes among its findings (Guberti 2021, PMID: 34202206). Absence of evidence isn't proof these claims are false, but they are not backed by the kind of trial data behind the respiratory and gut findings above — they're extrapolated from colostrum's growth-factor and IgG content, not tested outcomes.

One more distinction worth making: some of the strongest colostrum data involves reducing infectious diarrhea, particularly in children given hyperimmune colostrum from cows vaccinated against a specific pathogen — in one trial it shortened rotavirus diarrhea from a median of 72 hours to 56 hours (Mitra 1995, PMID: 8652974). Dairy allergy: a hard stop, not a caution to weigh. Colostrum is milk — it contains casein and whey, the proteins responsible for cow's-milk-protein allergy. A published case report describes a 16-year-old with a known milk allergy who had an anaphylactic reaction after a bovine-colostrum-containing cream was applied to a surgical wound, not even swallowed — confirmed by elevated whey-specific IgE antibodies (Porcaro 2019, PMID: 29549703). If you have a diagnosed milk allergy, avoid colostrum in any form, oral or topical. Lactose-intolerant people usually tolerate colostrum fine, since it's low in lactose — that's a separate issue from milk-protein allergy.

On that specific measure, colostrum helped after a single hard exercise bout, but an 8-week trial of daily colostrum during running training found permeability got worse, not better (a 251% increase versus a 21% increase with whey protein and a 7% decrease with no supplement at all). A 2024 meta-analysis pooling 10 studies found an overall improvement, but the studies disagreed with each other so much (I² = 99%, a measure of inconsistency) that the pooled number is not a reliable single verdict. This has nothing to do with the vague 'leaky gut' used in general wellness marketing, which isn't a measurable clinical outcome at all.

Does the IgG in colostrum survive digestion, or is it destroyed in your stomach?

A meaningful fraction survives, at least as far as your lower gut — but that's different from getting into your bloodstream. A human study using volunteers with an ileostomy found an average of 49% of an ingested dose of bovine immunoglobulin (up to 68% in people with fast gut transit) was recovered intact and still able to neutralize its target toxin by the time it reached the small intestine. A separate review of human studies found intact IgG recovered in stool ranges from trace amounts up to 25% of what was swallowed. That supports a local, in-the-gut mechanism. It is not evidence that oral colostrum delivers antibodies into your bloodstream the way an injection would — we're not aware of a human study showing that.

Does colostrum boost your immune system generally, or help skin and hair?

Not that trials have shown. The one immune-related finding with real trial support (fewer respiratory symptom days) is specific to athletes in heavy training, not a general immunity claim for a healthy, sedentary adult drinking a daily scoop. For skin, hair, and anti-aging, we are not aware of any published human trial testing bovine colostrum supplementation against those outcomes — the claims come from colostrum's growth-factor content, not from anyone having tested whether it actually changes skin or hair.

Who should not take colostrum?

Anyone with a cow's-milk-protein allergy. Colostrum is a dairy product containing casein and whey, the proteins that trigger milk allergy, and a published case report describes a teenager with a known milk allergy having an anaphylactic reaction after a colostrum-containing cream touched a wound — not even ingestion.
