The honest answer
Carnosine is literally beta-alanyl-L-histidine — but swallowing the intact dipeptide is an inefficient way to raise muscle carnosine. Oral carnosine is absorbed intact better than once assumed (Gardner 1991: up to ~14% recovered intact in urine), but an enzyme in blood, serum carnosinase, rapidly breaks the rest down into beta-alanine and histidine before it can meaningfully load muscle. Beta-alanine is the rate-limiting precursor muscle uses to make its own carnosine, and it isn't degraded by serum carnosinase the same way (Harris 2006) — the established, better-evidenced route to more muscle carnosine.
Same building blocks, very different route in
Carnosine (beta-alanyl-L-histidine) is a dipeptide your muscle stores and uses as a pH buffer during hard exercise. It is made of two amino acids joined together: beta-alanine and histidine. That chemistry is exactly why the "just take carnosine directly" logic seems intuitive — and exactly why it doesn't hold up. Gardner 1991, a human ingestion study, found that a real, non-trivial share of an oral carnosine dose — up to about 14% — is absorbed across the gut intact, which is more intact absorption than researchers had assumed for a dipeptide. But the same paper flags the problem: once that intact carnosine reaches the bloodstream, it runs into serum carnosinase, an enzyme that rapidly hydrolyzes it back into beta-alanine and histidine before much of it can reach and meaningfully load muscle tissue. Intact absorption is real; intact delivery to muscle is the part that mostly doesn't happen.
Why beta-alanine reaches the destination directly
Harris 2006 is the citation that resolves the question. This human dosing study established that orally supplied beta-alanine — not carnosine — measurably raises muscle carnosine concentration in human vastus lateralis (a thigh muscle), because beta-alanine is the rate-limiting substrate that muscle cells use to synthesize carnosine internally, via an enzyme called carnosine synthase. Critically, beta-alanine itself is not degraded by serum carnosinase the way the intact carnosine dipeptide is, so it reaches muscle and gets built into carnosine locally, inside the cell, rather than having to survive the bloodstream as an already-assembled molecule. The one carnosine trial that shows a performance effect — and why it doesn't change the verdict
Blancquaert 2021 is the trial most likely to be misread as evidence that oral carnosine works for performance after all. It's a real, double-blind, placebo-controlled crossover study with two sub-studies in healthy adults, and it found genuine effects: Study A found a 6% increase in initial 5-second Wingate power (12.8±2.4 vs 12.1±2.2 W/kg, p=0.04) after an acute pre-exercise dose of combined carnosine and anserine, and Study B found roughly a 3% increase in peak power across repeated tests, alongside a 2.5× rise in plasma anserine bioavailability when co-dosed with carnosine. Read the design carefully, though: this is an acute, single-dose trial measuring a brief window of elevated plasma availability immediately before exercise — not a chronic-dosing trial measuring resting muscle carnosine after weeks of supplementation, which is what the classic beta-alanine loading trials measure. And the trial's own mechanism discussion is explicit about why the effect is narrow and acute: it cites carnosine being "readily degraded in human plasma by the highly active serum carnosinase" as the underlying reason. In other words, Blancquaert 2021 doesn't refute the bioavailability problem — it's a demonstration of the bioavailability problem, caught in the brief window before serum carnosinase finishes its work. An acute plasma-availability bump before a single training session is a genuinely different claim from "raises your muscle carnosine store," which is the claim most people taking a carnosine supplement for exercise actually want.
