Does PQQ Actually Work? The Small-Trial, Single-Manufacturer Story

Three facts anchor this page, and none of them is the confident "boosts mitochondria" story you've probably read. The mitochondrial-biogenesis mechanism (PGC-1α/CREB) is real, replicated mouse and cell-culture biology (Chowanadisai 2010) — never measured as a human trial's primary outcome. The one human trial that measured a related biomarker in muscle (Hwang 2020) found it rose with zero functional benefit. And five of twelve human trials in this evidence base are Mitsubishi Gas Chemical-affiliated, with a sixth (the largest) tied to a second ingredient company — a level of industry concentration this page lays out tier by tier, honestly.

Three tiers of evidence, and why conflating them is the whole problem

PQQ marketing routinely blends three genuinely different kinds of evidence into one confident-sounding claim: a cell-culture mechanism, a cluster of small industry-linked human trials, and an animal-toxicology safety story. Separated honestly, they tell three different stories — and understanding where each one stops is the difference between an informed purchase and an oversold one.

Tier 1: Mitochondrial biogenesis (real, but mouse/cell-culture only)

PQQ activates CREB phosphorylation and increases PGC-1α expression, raising mitochondrial markers like citrate synthase, cytochrome c oxidase, and mtDNA, and protecting cells from mitochondrial toxins — demonstrated in mouse-hepatocyte (Hepa1-6) cell culture (Chowanadisai 2010, J Biol Chem). This is the foundational paper behind essentially every "PQQ boosts mitochondrial biogenesis" marketing claim on the internet. It is genuinely important, well-cited cell biology. It is also, without exception, cell-culture data in a non-human cell line — not a whole animal, and not a human. No trial in this evidence base has demonstrated mitochondrial biogenesis as a measured outcome of PQQ supplementation in a living human being. The single closest human data point actually argues for caution here: Hwang 2020 (n=23 untrained men, academic, non-industry-funded) measured PGC-1α protein in human muscle after 6 weeks of PQQ plus endurance training, and it rose significantly in the PQQ group — a real biomarker signal, in humans, in a non-industry trial. But the same trial found NO additional aerobic-performance or body-composition benefit over training alone. The mechanism moved. The outcome didn't. That is the single cleanest lesson in this entire evidence pack about the gap between a biomarker and a proven benefit.

Tier 2: Human trials (small, and concentrated around two ingredient companies)

PQQ's human evidence is a consistent-but-small cluster of BioPQQ-branded, Mitsubishi-Gas-Chemical-linked RCTs covering cognition (Itoh/Nakano 2016), cerebral blood flow (Nakano 2016), cholesterol (Nakano 2015), and age-split memory (Tamakoshi 2023) — five of twelve trials total, all tracing to the company that manufactures BioPQQ™. A sixth, the largest completer-n trial in this pack (Shiojima 2022, n=58), tested a different branded ingredient (mnemoPQQ®) and has a senior co-author who is the Chief Scientific Officer of Cepham Inc., a supplement-ingredient company — industry-linked too, just a different company and molecule, and not an independent replication of the BioPQQ cluster. That leaves exactly two genuinely independent, non-industry human data points in the whole pack: Harris 2013 (UC Davis, Rucker lab, n=10, crossover pilot) found PQQ lowered inflammatory markers (CRP, IL-6) and altered urinary markers consistent with mitochondrial-related metabolism — a real independent biomarker signal, but n=10 and no cognitive or clinical outcome measured. And Hwang 2020, discussed above. Two small, non-industry, biomarker-only trials is a thin independent counterweight against six industry-linked trials that cover the more attention-grabbing cognition and cerebral-blood-flow claims.

Tier 3: Safety (well tolerated in humans; a much older, different-route rat signal)

Every human trial in this pack reports PQQ as well tolerated, up to 21.5mg/day for 12 weeks, with no serious adverse events. On the kidney question specifically, three animal studies tell a story that needs all three read together, not any one alone. A 1989 study (Watanabe 1989) found proximal tubular damage in rats — but used intraperitoneal injection, not oral dosing, over just 4 days at 11.5mg/kg/day, a different route and a much older, smaller study than what follows. Two modern, larger, ORAL toxicology studies supersede it for real-world supplement relevance: Nakano 2014 (Mitsubishi Gas Chemical-affiliated, Sprague-Dawley rats, 13 weeks) found a no-adverse-effect level of 100mg/kg/day, with reversible, high-dose-only kidney changes in females; and Liang 2015 (an independent Chinese toxicology group, 13 weeks) found no adverse effects at any dose up to 400mg/kg/day. Both oral margins are roughly 300-1,500× the human trial dose — genuinely reassuring, but that margin describes animal toxicology, not license for a much higher human dose than any trial has actually tested.

The verdict PQQ has a real, if narrow, mitochondrial-biology story and a small, low-diversity human evidence base that mostly traces back to the company that sells the branded ingredient. That doesn't make it fake — it makes it exactly the kind of evidence that needs independent replication before it earns a bigger claim.

Why this matters more than the usual "more research needed"

Plenty of supplements get a generic "promising, needs more research" writeup. PQQ is a sharper case in one specific way: the mechanism that justifies its marketing (mitochondrial biogenesis) and the human trials that generate its buzz (cognition, cerebral blood flow) are, respectively, never tested in humans at all and mostly tested by the company selling the ingredient. That's a materially different evidence state than "nobody has looked yet" — researchers have looked, repeatedly, and mostly from inside the two companies with a financial stake in the answer. It doesn't mean PQQ supplementation does nothing; a consistently-replicating small-trial signal, even an industry-funded one, is not nothing, and well-run RCTs don't become invalid because of who paid for them. But it does mean the honest framing has to lead with the concentration and the mechanism gap, not bury them under the more exciting cognition or "boosts mitochondria" headlines the field itself uses to sell the ingredient.
