MCT Oil vs Coconut Oil: Not the Same, and the Ketone Data Proves It

Coconut oil is roughly 50–65% MCTs by weight, but the fatty acid that actually drives ketosis — C8/caprylic acid — makes up only about 6–8% of coconut oil. Lauric acid (C12), the least ketogenic MCT, dominates instead. A direct head-to-head trial (Norgren 2020) found C8 alone produced a significantly higher ketone response than whole coconut oil, and coconut oil was not significantly different from a non-MCT sunflower-oil control on several comparisons. "MCT oil" — especially a C8-isolated product — is a concentrated, purpose-refined product for the ketone effect specifically. Coconut oil is a food that happens to contain some MCTs. They are not interchangeable, despite near-identical marketing language.

Why "coconut oil" and "MCT oil" get used interchangeably (and shouldn't be)

The confusion has a real basis: MCT oil is typically refined FROM coconut or palm kernel oil, and coconut oil genuinely does contain medium-chain triglycerides — roughly 50–65% of its fatty acid content by weight. That's enough overlap that marketing copy routinely treats the two as synonyms, or implies that eating coconut oil delivers the same ketogenic punch as taking MCT oil. It doesn't, because the fatty acid composition inside that 50–65% matters enormously. Coconut oil's MCT content is dominated by lauric acid (C12) — and C12 is, per St-Pierre 2019's direct comparison, the least ketogenic of the medium-chain fatty acids, roughly 6x less ketogenic than C8 at an identical dose. Caprylic acid (C8) — the fraction that drives MCT oil's ketone effect — makes up only about 6–8% of coconut oil. A tablespoon of coconut oil and a tablespoon of a C8-concentrated MCT oil are simply not delivering comparable amounts of the compound that matters.

The head-to-head test: Norgren 2020

This isn't an inference from fatty-acid percentages alone — it's been tested directly. Norgren 2020 ran a 6-arm cross-over study in n=15 healthy older adults (65–73y) comparing C8 (20g), coconut oil (30g), and a sunflower-oil control, specifically designed to separate "is this a generic dietary-fat effect" from "is this a C8-specific effect." The result: C8 had a significantly higher ketone (BHB) AUC than either coconut oil or the sunflower-oil control (p≤0.02 for those comparisons) — and coconut oil was not significantly different from the sunflower-oil control. In other words, in this trial, coconut oil didn't behave like a meaningfully ketogenic MCT source at all; it performed statistically like a non-MCT control fat. That's the single most direct piece of evidence in this category that "coconut oil" and "MCT oil" are not the same product for this purpose.

The metabolism claim also failed a direct test

The other half of coconut-oil marketing — "boosts your metabolism," "burns fat faster" — has its own direct null result. LaBarrie & St-Onge 2017 gave n=15 obese adolescents a coconut-oil-enriched meal versus a corn-oil meal in a randomized cross-over design and measured thermogenesis, appetite/satiety, glucose, and insulin over 6 hours: no significant difference on any measure. This is specifically about coconut oil, not isolated C8, and it's the honest counterweight to the "coconut oil boosts metabolism" claim that circulates independently of the MCT-oil conversation — tested directly, and it didn't hold up.

The verdict Coconut oil is a food with some MCT content, dominated by the least-ketogenic chain length (C12). "MCT oil," especially a C8-isolated product, is a concentrated, purpose-refined product built around the fraction that actually drives the ketone effect. A head-to-head trial found C8 significantly more ketogenic than coconut oil, and coconut oil statistically indistinguishable from a non-MCT control. They share a supply chain, not a physiological effect.
