Does HMB Work? The Trained-Athlete Hype vs the Independent Evidence

The honest answer

The famous "12 weeks of HMB added 16 lbs of lean mass" study was funded by and co-authored with the company that patented and sells HMB (Wilson 2014) — and when independent researchers with no financial stake ran the same kind of trial in trained athletes, they found no effect (Sanchez-Martinez 2018: "no effect of HMB supplementation on strength and body composition in trained and competitive athletes"). This page lays out both sides in full, so the magnitude of the gap is visible, not just the conclusion.

The headline numbers, and the funding behind them

Wilson et al. 2014 ran a 3-phase RCT (8-week periodized resistance training, 2-week overreaching cycle, 2-week taper) in 20 completers (of 24 randomized), young resistance-trained men, at 3g/day HMB-FA vs. placebo. The numbers were striking: total strength (squat+bench+deadlift) up 77.1±18.4kg vs. 25.3±22.0kg for placebo (p<0.001); vertical jump power up 991±168 vs. 630±167 W (p<0.001); lean body mass up 7.4±4.2kg vs. 2.1±6.1kg (p<0.001) — roughly 3.5x the placebo group's gain, in 12 weeks, a magnitude far outside what's typically reported for any legal oral supplement in trained lifters. Lowery et al. 2016, from the same University of Tampa research group, reported similarly outsized numbers for an HMB-FA-plus-ATP combination: lean body mass +12.7%, strength +23.5%, jump/Wingate power +21.5%/+23.7% (all p<0.001) — though this trial cannot cleanly isolate HMB's own contribution from the added ATP.

Both trials share the same funding and authorship pattern, and it is not a footnote. Wilson 2014's conflict-of-interest statement, quoted directly from the open-access full text: "This research was funded in part through a grant from Metabolic Technologies Inc. ... JR, JF, and SB are employed by Metabolic Technologies, Inc." (Rathmacher, Fuller, and Baier — all Metabolic Technologies Inc./Iowa State University Research Park, the company holding commercial interests in HMB sales). Lowery 2016 lists MTI-employed co-authors Rathmacher and Fuller directly in the byline, with the affiliation "Metabolic Technologies, Inc., Iowa State University Research Park, Ames, Iowa" stated plainly. No independent replication of this magnitude of effect exists anywhere in the evidence reviewed for this cluster.

The patent-holder's own meta-analysis

The story traces back further. Nissen et al. 1996 is the field's founding paper — author Steven Nissen, then at Iowa State University's Department of Animal Science, discovered HMB reduced exercise-induced muscle proteolysis and increased fat-free mass in early resistance-training studies. Nissen went on to co-found Metabolic Technologies Inc. and hold the commercial patents that underlie HMB's entire supplement category. His 2003 follow-up, Nissen & Sharp 2003, a meta-analysis spanning 250 supplements from 1967–2001, found creatine and HMB both significantly increased lean mass (0.36%/wk and 0.28%/wk respectively) and strength (1.09%/wk and 1.40%/wk, both p<0.05) while chromium, DHEA, androstenedione, and protein did not. This is the field's own patent-holder analyzing his own product's literature — never cite this meta-analysis without disclosing that conflict in the same sentence, which is exactly why it's named here rather than treated as neutral supporting evidence.

The independent counter-evidence, in the same population

When researchers with no MTI tie ran meta-analyses in the same trained-athlete population, they found essentially nothing. Rowlands & Thomson 2009, an independent meta-analysis (Massey University, New Zealand), pooled 9 qualifying studies (14 comparisons, n=394 subjects, 5±2 weeks intervention) and found: "There were small benefits to lower-body (9.9% ± 5.9%) and average strength (6.6 ± 5.7%)... in untrained lifters. In trained lifters, all strength outcomes were trivial." The combined overall strength increase across all studies was "trivial (3.7 ± 2.4%)," effects on fat and fat-free mass were "trivial," and the authors' own conclusion states plainly: HMB's "effect on body composition is inconsequential."

Sanchez-Martinez et al. 2018 is the most direct, most current rebuttal in this evidence base — an independent meta-analysis (Chile/Spain universities, no MTI affiliation) of 6 RCTs, n=193 trained/competitive athletes, using a fixed-effect model with low heterogeneity (I²=0.0%). The population and outcome measures most closely mirror what Wilson 2014/Lowery 2016 claimed to show. The result: "HMB supplementation interventions present a trivial non-significant ES in all variables studied (bench press ES=0.00, leg press ES=0.09, body mass ES=-0.01, fat-free mass ES=0.16, and fat mass ES=-0.20; all cases p>0.05)." The authors' conclusion, quoted directly: "This meta-analysis found no effect of HMB supplementation on strength and body composition in trained and competitive athletes."

A softer conflict pattern worth naming once

Beyond direct MTI employment, an authorship-overlap pattern runs through the more HMB-favorable reviews in this evidence base. Wilson JM — lead author on the industry-funded 2014 trial — is also lead author on Fitschen et al. 2013, a narrative review of HMB in elderly/clinical populations that states "many studies have found increases in lean mass and strength... however, not all studies have found beneficial effects" — a more favorable framing than the fully independent reviews reach, read here with that authorship overlap in mind. Similarly, co-author Stout JR connects the industry-funded trial lineage to Silva et al. 2017, a systematic review whose own title hedges ("may improve recovery and muscle adaptations") while still landing on a more favorable read than Rowlands 2009 or Sanchez-Martinez 2018. This pattern is worth naming once, clearly, rather than re-litigating on every citation: research groups with a stake in HMB's commercial success, directly or through repeat co-authorship, consistently report more favorable conclusions than fully independent groups testing the same claims.

HMB vs. creatine and leucine: the honest comparison

If you want the sports supplement in this family with the deepest, most consistently independently-replicated evidence base, it's creatine, not HMB — creatine's strength and power benefits have been reproduced across hundreds of independent trials with no comparable industry-funding-versus-independent-replication gap. And because HMB is a downstream metabolite of leucine, it's worth noting Wilkinson et al. 2013's direct head-to-head: at an equal gram dose, leucine alone produced a larger acute muscle-protein-synthesis response (110%) than its own downstream metabolite, HMB (70%) — a real argument that plain leucine, or simply adequate dietary protein (which already supplies leucine thresholds for most people), may be the more efficient lever than an isolated HMB supplement. The one-line takeaway HMB is the weakest-evidenced member of this supplement family for a trained lifter specifically: its headline numbers come from MTI-funded trials, and the most direct independent tests of the same claim found nothing.
