Glutathione: What Trials Show About Absorption and Skin Lightening

Two separate honest questions, and they get two different answers. Absorption: the old "oral glutathione is destroyed in the gut" claim is overstated — Richie 2015's 6-month RCT (n=54) found 250mg and 1000mg/day raised body glutathione stores dose-dependently, real and solid. Skin lightening: small, short oral RCTs (Arjinpathana 2012, Weschawalit 2017) show a genuine but thin signal, positive yet limited to 4-12 weeks. IV glutathione is a different story entirely: no RCT efficacy evidence, not FDA-approved for skin lightening, and the Philippine FDA issued a public safety warning after adverse effects (Sonthalia 2016). What glutathione is, and three claims that get conflated

Glutathione is a small molecule made from three amino acids (cysteine, glutamate, glycine) that your body produces on its own and uses as its master intracellular antioxidant — a real, textbook biochemical role, not a marketing invention. It's not an essential nutrient in the vitamin sense: there's no deficiency state, no RDA, and no government-set upper limit, so any claimed benefit of supplementing above what your body already makes has to stand on its own evidence. Glutathione marketing routinely blends three genuinely different claims into one confident pitch, and separating them is the entire point of this cluster. First, does oral glutathione raise your body's glutathione stores? Yes, and this is the best-supported claim here — Richie 2015 measured blood, erythrocyte, plasma, lymphocyte, and buccal-cell glutathione directly, over 6 months, in a dose-dependent RCT. Second, is oral glutathione a proven skin-lightening treatment? Positive, but thin — two small RCTs (Arjinpathana, Weschawalit) and one uncontrolled, single-arm open-label trial (Handog), all 4-12 weeks, none establishing long-term safety or a large effect. Third, is a specific delivery form (liposomal, S-acetyl, sublingual) more bioavailable than standard capsules? Largely unverified in humans — the one human head-to-head (Schmitt 2015) actually found sublingual beat standard oral, liposomal's edge is a rat-only finding, and S-acetyl has no published human absorption data at all. A strong finding on one of these three claims does not lend credibility to the other two, and this cluster never lets that happen.

Does it work? (the honest short version)

For raising your own glutathione levels: yes, reliably, at 250-1000mg/day over months, per Richie 2015 — this is real pharmacology with a clean dose-response curve. For skin lightening: modestly and short-term, in small trials that their own authors describe carefully. Arjinpathana's own conclusion, quoted directly, is that oral glutathione "results in a lightening of skin color in a small number of subjects" — not a dramatic or guaranteed effect, and long-term safety was explicitly not established. Handog's positive open-label result carries real limitations: no control group, no disclosed dose, and effects that can't be separated from expectation or measurement bias without a comparison arm. For "which form absorbs best": the honest answer is that nobody has run the human trial that would settle it. Schmitt 2015's finding that sublingual outperformed standard oral capsules is the only human head-to-head evidence in this pack, and it argues against, not for, the plain-capsule route being optimal — while liposomal and S-acetyl marketing claims currently rest on animal data or mechanism alone.
