Does Citrus Bergamot Lower Cholesterol? An Evidence-Quality Reckoning

The honest answer
Probably yes, to some real but modest degree — but the size of the effect you'll read about depends heavily on who ran the trial. Trials from groups with commercial or institutional ties to bergamot report LDL drops up to 40%+. The one clearly independent systematic review (Lamiquiz-Moneo 2020, 12 studies) found real signal but called the underlying study quality "quite limited." One independent trial found no effect at all at a lower dose. This is nowhere near statin-level outcome evidence — no trial has measured a heart attack, stroke, or mortality endpoint, only surrogate lipid markers.

The four industry/institution-linked rows report the largest, most dramatic effects in the literature. The two independent rows — a systematic review and a null trial — tell a more cautious story. Both belong in the same summary; neither should be cited alone.
What the independent review actually found
Lamiquiz-Moneo 2020 is the load-bearing citation for this entire cluster. Screening 442 studies down to 12 that met eligibility criteria, it found 75% showed a significant decrease in total cholesterol, triglycerides, and LDL-C, with effect ranges spanning TC −12.3% to −31.3%, LDL-C −7.6% to −40.8%, and TG −11.5% to −39.5%, and 8 of 12 trials also reporting an HDL-C increase. That's a real, positive signal from a genuinely independent source (a Spanish lipid and atherosclerosis unit with no financial or institutional connection to bergamot). But the review's own authors state plainly: "studies had heterogeneous designs and scientific quality of studies was quite limited." Read literally, that means the review's authors — the people who read all 12 underlying trials most carefully — do not consider this settled, high-quality evidence, even while finding a real signal.
Combination products: a separate, easily confused category
Two trials in this evidence pack test bergamot as part of a multi-ingredient formula, not alone, and their results should never be attributed to bergamot by itself. Fogacci 2024 (n=90, double-blind RCT) tested a combined artichoke-plus-bergamot supplement and found significant lipid improvements — a real result for that combination product, with no way to isolate bergamot's individual contribution. Fogacci 2022 (the ANEMONE study, n=60 RCT) tested Eufortyn® Colesterolo Plus, a formula combining bergamot polyphenolic fraction phytosome, artichoke extract, CoQ10 phytosome, and zinc, funded by the product's manufacturer (Scharper S.p.A., "no role" in study conduct disclosed) — again a real finding for that specific combination, not evidence for bergamot alone. Marketing that cites either trial's effect size as "what bergamot does" is misattributing a combination-product result to a single ingredient.
The bergamot-vs-red-yeast-rice honesty check
One of the more useful independent data points in this evidence pack isn't a bergamot trial at all — it's a comparative review. Kłosiewicz-Latoszek 2021, from Poland's National Institute of Public Health, reviewed the nutraceutical cholesterol-lowering landscape and concluded that "red yeast rice and Armolipid are the most favourable for reducing cardiovascular events. However, the evidence of benefits of berberine and bergamot is not so conclusive," adding that "the opinion of experts on berberine and bergamot is ambiguous." That's a direct, independent ranking that places bergamot in a clearly lower evidence tier than red yeast rice — useful context for anyone comparing nutraceutical options rather than assuming all "natural cholesterol supplements" carry equal evidentiary weight. A broader review (Cicero 2021) takes a more favorable overview tone toward bergamot and other nutraceuticals, but its authors disclose extensive pharma/nutraceutical industry ties, including grants from companies that sell bergamot-combination products covered in this pack — weight it accordingly. A more institutionally independent overview (Arnaboldi 2022, University of Milan pharmacology department) is supportive of standardized bergamot/artichoke extracts for mild-to-moderate dyslipidemia, but like the other two reviews cited here, it is a narrative synthesis of existing data, not a new primary trial.
The verdict Bergamot polyphenols plausibly lower LDL, triglycerides, and raise HDL — that's a real, independently-supported signal, not pure marketing. But the literature is dominated by small, short, single-country, often industry- or institution-linked trials, the one clearly independent systematic review calls study quality "quite limited," and an independent comparative review ranks bergamot's evidence below red yeast rice. This is nowhere near statin-level outcome evidence: no trial has measured a fracture, heart attack, or mortality endpoint — only surrogate lipid markers over 30 days to 6 months.
