Wang et al. 2011 (Nature) established the mechanism — germ-free mice, lacking the gut bacteria to make TMAO, did not develop the same atherosclerosis-promoting effect from dietary phosphatidylcholine. Tang et al. 2013 (New England Journal of Medicine) is the strongest human data: in a cohort of 4,007 patients followed 3 years, fasting plasma TMAO in the highest quartile carried a hazard ratio of 2.54 for major adverse cardiovascular events. The honest caveats matter as much as the headline number: this cohort was referred for cardiac catheterization, meaning its baseline cardiovascular risk was already elevated compared to the general population, and the finding is an observational association, not a randomized trial showing that choline supplementation at Adequate-Intake-range doses causes cardiovascular harm in healthy people. It is dose-relevant and individual-microbiome-dependent. The honest read: TMAO is a legitimate reason for extra caution around supraphysiologic bitartrate dosing, especially if you already have cardiovascular risk factors — it is not a reason to avoid food-source or Adequate-Intake-range choline, and it should never collapse into a flat "choline causes heart disease" claim.
