Ashwagandha Benefits: What the Evidence Actually Shows

Not every ashwagandha benefit has the same amount of evidence behind it, and most of what's claimed is dose- and extract-specific. Stress and cortisol reduction is the best-evidenced use: a randomized trial at 600mg/day for 60 days cut cortisol 27.9% from baseline versus 7.9% for placebo. Sleep quality has strong meta-analysis support at the same dose. Anxiety symptoms improve moderately and consistently, but the effect is smaller than the stress result. Testosterone and strength claims are the weakest of the group — they come from one small trial with a manufacturer-supplied extract and no independent replication, and a second trial found the same trend but it missed statistical significance against placebo. None of this is immediate: the trials behind these numbers ran 8 weeks or longer.

Why the extract on the label decides which benefits apply to you

Every benefit described below came from a trial using a specific, standardized extract at a specific dose — not from "ashwagandha" as a generic ingredient. Most of the strongest results (cortisol, testosterone, strength) come from trials of KSM-66, a full-spectrum root extract standardized to 5% withanolides. Some of the sleep and anxiety evidence comes from Shoden, a higher-potency extract standardized further and typically dosed lower (around 240mg/day rather than 600mg/day). Sensoril, a root-and-leaf extract, appears in some of the older sleep-focused literature. Generic, unstandardized "ashwagandha root powder" — common in cheap capsules — has essentially none of this trial backing at any defined dose, because it isn't what any of these studies used.

Stress and cortisol: the best-evidenced benefit

This is where ashwagandha's evidence is strongest. In a randomized, double-blind, placebo-controlled trial, 64 adults with chronic stress took 300mg of a high-concentration full-spectrum ashwagandha extract twice daily (600mg/day total) for 60 days. Serum cortisol — the hormone your adrenal glands release under sustained stress — dropped 27.9% from baseline in the ashwagandha group versus 7.9% in the placebo group, a statistically significant between-group difference (p=0.0006), alongside significant improvements on multiple stress questionnaires (Chandrasekhar 2012, PMID: 23439798). The trial reported no funding conflict and no industry support.

That result isn't a one-off. A 2022 systematic review and meta-analysis pooling multiple randomized trials of ashwagandha for stress and anxiety concluded the supplement produced a meaningful reduction in stress and anxiety measures versus placebo (Akhgarjand 2022, PMID: 36017529). When an individual trial's effect size holds up across a pooled analysis of several independent studies, that's a stronger evidence signal than any single paper on its own.

Sleep: real, but it isn't a sedative

A systematic review and meta-analysis of randomized controlled trials found ashwagandha significantly improved sleep quality measures versus placebo — and the authors specifically noted the effect was clearest at 600mg/day, taken for at least 8 weeks, and in participants with diagnosed insomnia rather than healthy sleepers with no sleep complaints (Cheah 2021, PMID: 34559859). The mechanism appears to be the same cortisol-lowering pathway behind the stress result above, not direct sedation — which is why it builds gradually rather than knocking you out on night one. Anxiety: moderately well-evidenced, but a smaller effect than stress

Anxiety and general stress overlap but aren't identical, and the anxiety-specific evidence is a notch below the cortisol result above. In a separate 60-day randomized trial, 60 stressed adults took 240mg/day of Shoden, a different standardized extract, and showed a statistically significant reduction on the Hamilton Anxiety Rating Scale (p=0.040), a clinician-administered anxiety measure, alongside significant drops in morning cortisol and DHEA-S — but the reduction on a second self-report stress scale (DASS-21) only reached near-significance (p=0.096) (Lopresti 2019, PMID: 31517876). That mixed result — one measure clearly significant, a second one not quite — is typical of the anxiety literature: the pooled 2022 meta-analysis above found a real effect across studies, but the individual trials it drew from used different extracts, doses, and durations, which is why the effect reads as moderate rather than as strong as the cortisol number. None of this is a substitute for treatment of a diagnosed anxiety disorder.

Testosterone and strength: the weakest of these claims

This is the one to be most skeptical of. In an 8-week randomized trial, 57 young men with little resistance-training experience took either 300mg of KSM-66 twice daily (600mg/day) or placebo alongside a training program. The ashwagandha group had significantly greater increases in bench-press 1-rep max (+46.0kg vs +26.4kg, p=0.001), leg-extension strength (+14.5kg vs +9.8kg, p=0.04), and serum testosterone (+96.2 ng/dL vs +18.0 ng/dL, p=0.004) (Wankhede 2015, PMID: 26609282). That sounds impressive, but the KSM-66 extract used in the trial was supplied free by its manufacturer, the sample was small (57 total, split across two arms), and no independent replication of this specific testosterone/strength finding turned up in this review.

The testosterone signal is also inconsistent across the literature: in the separate Shoden trial cited in the anxiety section above, testosterone rose in male participants over the study period, but that change was not statistically significant compared with placebo (p=0.158) (Lopresti 2019, PMID: 31517876). One trial found a clear effect; another, using a different extract, found the same direction of change but it didn't clear the statistical bar. That combination — one positive small industry-linked trial, one null result on the same hormone — is exactly the profile of a preliminary finding, not a proven benefit. If building strength or raising testosterone is your main goal, don't treat ashwagandha as a substitute for training and sleep, which have far larger and more consistent effects on both.

Read that as "an early trial found a testosterone/strength benefit worth testing further," not proof it will happen for you.

How long until I'll feel ashwagandha's benefits?

Not immediately, for any of these benefits. The cortisol trial ran 60 days (about 8.5 weeks) before measuring its result. The sleep meta-analysis found the clearest benefit specifically at 600mg/day for 8 or more weeks. The anxiety and testosterone trials ran 8 weeks as well. Ashwagandha works by gradually shifting a hormonal baseline, not by acting like a fast-acting calming pill or stimulant — judge it at 8 weeks of consistent daily use at a studied dose, not after a few days.

Does the extract type change which benefits I actually get?

Yes, and this is the detail most articles skip. "Ashwagandha" on a label can mean a KSM-66 full-spectrum root extract (5% withanolides, the extract behind most of the cortisol, strength, and testosterone trials), a Sensoril or Shoden root-and-leaf or root extract standardized to a higher withanolide percentage (used in some of the sleep and anxiety trials, often at a lower mg dose), or unstandardized generic root powder with no defined withanolide content and essentially no trial evidence behind it. A product that doesn't name its extract can't be matched to any of the studies above, no matter what the label claims.
