"methodology": "D-mannose is itself the active ingredient (unlike cranberry, where the active PAC is a separate, often-undisclosed fraction). The studied prevention dose is 2 g/day, used identically in both controlled trials that matter: Kranjcec 2014 (PMID 23633128) and MERIT 2024 (PMID 38587819), both dosed as powder dissolved in ~200 mL water once daily. mgPerServing = disclosed D-mannose mg at the label's stated serving; costPerDayAt2g = cost to reach the full 2 g dose (null where a product's D-mannose content is not independently confirmed, excluding it from the ranking). Most capsule products are 500 mg/capsule, so 4 capsules are needed to match one trial dose -- capsulesFor2g flags this. EVIDENCE CONTEXT (structure/function only): MERIT (Hayward 2024, JAMA Intern Med, 598 women, double-blind placebo-controlled, 99 UK primary-care sites) found NO significant reduction in UTI-related ambulatory-care contacts vs placebo (51.0% vs 55.7%, p=.26) and concluded D-mannose \"should not be recommended for prophylaxis.\" Cochrane (Cooper 2022, PMID 36041061) rated the pre-MERIT evidence as low-quality/insufficient. D-mannose is NOT a treatment for an active/symptomatic UTI -- antibiotics are needed; anyone with fever, flank pain, or nausea/vomiting should seek prompt clinical care. This statement has not been evaluated by the FDA; these products are not intended to diagnose, treat, cure, or prevent any disease.",
